    Employment Opportunity: Grey Bruce Ontario Health Team – Director, OHT Implementation

    The Director, OHT Implementation is the administrative lead for the Grey Bruce Ontario Health Team (OHT) reporting directly to the OHT Partnership Table and Co-Chairs. The Director, OHT Implementation will manage a growing team of OHT staff and support clinical and community leaders as this 'cohort 3' OHT continues to mature. The Director will act as the primary GB OHT liaison with Ontario Health (OH) and will collaborate with OHT leaders across OH West and provincially.

    The ideal candidate will be comfortable managing multiple priorities simultaneously, engaging with a wide variety of key interested parties across the health care system. The Grey Bruce OHT's upcoming 3-Year priority plan will define the local priorities for OHT partners from 2024 to 2027. The Director, Implementation will support the OHT Partnership Table, related Councils and evolving OHT Action Teams.

    We are seeking an excellent facilitator, communicator and system leader who will ensure that patient/client and caregiver partners are central to planning and decision-making. The candidate will be skilled providing the GB OHT with operational and administrative leadership that enables the OHT to successfully meet the following mandatory OHT deliverables:

  • Integrated Care through Population Health Management (PHM) and Equity Approaches
  • Patient Navigation
  • Readiness for Home Care Delivery
  • Collaborative Leadership, Decision Making and Governance, including future OHT incorporation
  • Primary Care Engagement and Leadership
  • Digital and Information Management
  • Collaborative Quality Improvement Plan
  • Qualifications

  • Master's degree in public administration, business administration, healthcare administration or project management, or equivalent education and experience;
  • Minimum 5 years of senior leadership experience in clinical/operations in a healthcare environment such as hospitals, long term care, primary care, government or private sector required;
  • Demonstrated knowledge and achievement in health system planning, evaluation and approaches to integrated care;
  • Demonstrated ability to guide, coach and manage large, complex teams with multiple stakeholders;
  • Demonstrated ability to develop effective and comprehensive reports and presentations for various audiences, including patients and their families, community members, executives, administrators and clinicians;
  • Strong understanding of Grey Bruce communities and familiarity with capacity planning analytics, solutions and partnerships;
  • Superior analytical, problem-solving abilities and consensus building facilitation skills;
  • Ability to work under pressure and tight timelines;
  • Demonstrated innovative and out-of-the-box thinking and ability to handle and navigate ambiguity;
  • Extensive knowledge of provincial health care structures, processes, funding mechanisms and current priorities is strongly preferred; and
  • Working knowledge of health system financial modeling and financial analysis is preferred.
