Position Summary


The Social Worker will be an integral member of the Toronto Western Family Health Team - Garrison Creek site (TW FHT - GC).

The TW FHT - GC provides primary health services, using multi-disciplinary and inter-professional team approaches to a variety of patient populations, which include: a) underserved, at-risk and high needs patients, b) elderly/home bound patients, c) adolescents and emerging adults and d) patients with mental health and addictions.


The Social Worker will work as an integrated member of the inter-professional care team and will provide psychosocial assessments, psychotherapy, counseling, and health promotion services to TW FHT - GC patients.

The Social Worker will function at a high level of professional autonomy and will demonstrate expert skills to formulate clinical decisions, and patient-centred care for TW FHT patients and their families.

The Social Worker works in a collaborative practice and consultation models with various other TW FHT primary health care providers.

The Social Worker will also be integral to organizational and community planning, including building partnerships with community resources/agencies, program development, and inter professional education and social work education.

The Social Worker will demonstrate commitment to continued professional development, critical decision making and patient-centered care, and will perform other duties consistent with the job classification, as required.


Qualifications

  • Successful completion of a Masters Degree in Social Work (M.S.W.) or recognized equivalent required
  • Membership with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) required
  • Three (3) years experience working with patients and families in a multidisciplinary team within a primary care setting preferred
  • Experience with the elderly, adolescents and emerging adults, immigrant population or mental health/addictions patients
  • Experience providing psychosocial assessments, brief counseling, psychotherapy and psychoeducational interventions (e.g. cognitive behavioural therapy; solutionfocused therapy; crisis counseling, and motivational interviewing)
  • Experience with systems navigation with complex/vulnerable patient populations preferred
  • Experience with group based interventions
  • Experience in building community partnerships for the enhancement of patient services and continuity of care
  • Experience in educating health professionals and social work preferred
  • Program planning, quality improvement and quality assurance interest/experience required
  • Commitment to patientcentred care with the ability to adapt to a diverse patient populations
  • Excellent interpersonal, verbal and written communication skills
  • Demonstrated ability to work effectively as an inter professional health care team member
  • Excellent organizational, time management and problem solving skills
