As a member of the Professional Practice Portfolio at SE Health, collaborating with Operations leadership, the Professional Practice Leader-Rehab is responsible for achieving excellence in patient safety, professional practice and clinical quality with a focus in evidence-based rehabilitation (rehab) services in a variety of settings across Canada (e.g.

home/community, assisted living, long term care, etc.).


JOB RESPONSIBILITIES:


Activity A:
Practice and Compliance


  • Provide expert knowledge and professional practice leadership to develop solutions/service delivery models for exceptional care rooted in evidenceinformed practice.
  • Set practice standards, policies, and procedures in a variety of clinical settings across Canada.
  • Evaluate clinical quality, risk, and outcomes for continuous quality improvement and risk mitigation.
  • Adopt evidenceinformed leading practices and provide expert knowledge to advance care and meet needs of the population served
  • Monitor federal, provincial, and other relevant legislation/regulations for compliances and collaborate on strategies to address opportunities for improvement
  • Conduct regular review and analysis of clinical quality and outcome indicators.

Activity B:
Developing Education programs


  • Provide support and consultation
  • Collaborate to address quality, safety, and risk management issues, including the development of improvement plans to address opportunity areas.
  • Develop and contribute to execution of educational initiatives and support the ongoing development of our innovative online learning environment.

Activity C:
Leadership and Development


  • Lead program development and new models of care delivery for the target population, in collaboration with the operations leadership team and the interdisciplinary team.
  • Support Rehab Practice Coaches through regular meetings, orientation, mentorship, consultation, and guidance with complex clinical situations.
  • Lead or participate in knowledge translation, quality improvement, risk management, research, and best practice initiatives.
  • Provide clinical and thought leadership and collaborate with a diverse range of internal and external stakeholders to achieve the defined mandate.

Activity D:


  • Represent the organization through conference participation, presentations, and publications.
  • Committee participation and leadership as required.

QUALIFICATIONS:


  • Demonstrated core competencies in an advanced rehab practice capacity in the domains of Clinical, Collaboration/Consultation, Research/Evidence, and Leadership.
  • Registration with one of the rehabilitation discipline regulatory bodies in Ontario in good standing: Physiotherapy, Occupational Therapy, Social Work, Speech-Language Pathology, Dietitian Master's degree in one of the rehabilitation disciplines preferred.
  • Membership in relevant provincial and national associations/interest groups an asset
  • Rehabilitation leadership experience is an asset
  • Minimum of three (3) years of practice experience, home and community experience preferred.
  • Solid understanding of principles of evidence informed practice, adult learning, and change management.
  • An ability to promote and facilitate evidencebased practice.
  • Project management experience in executing on evidence informed practices and quality improvement initiatives.
  • Demonstrated leadership ability, including facilitation, engagement, conflict resolution, problem solving and critical thinking skills.
  • Demonstrated expertise and ability to apply, teach, coach and mentor individuals and teams
  • Demonstrated success in establishing, leading and maintaining positive change through the use of change management principles.
  • Excellent organizational, planning and time management skills, as well as the ability to manage multiple deliverables.
  • Ability to be resilient within a changing environment.
  • A demonstrated commitment to continuous learning and improvement.
  • A vehicle, travel, and some oncall may be required.
  • Work location is hybrid to Markham and will travel to sites as required.
