    Responsibilities:

  • Reporting to the Program Manager
  • Supports the completion of RAI-MH Assessments
  • Assumes responsibility for the computerized data submission process to CIHI and maintains records following each submission within the time frames outlined by the Ministry of Health and Long Term Care
  • Coordination of timely and correct data input by clinical staff for data analysis and trending to promote Quality Improvement initiatives
  • Monitors and assists with the completion as needed. Completes hands on assessments of patients for initial, quarterly, change-in-status, and discharge submissions
  • Responsible to upload all the assessments submitted to OMHRS to the LHIN (IDS) portal
  • Provide education and training to all professionals in RAI-MH completion
  • Collect statistics as required
  • Work with Performance Analytics to submit monthly statistical reports to Program Leadership
  • Critically analyzes statistical data in relation to the clinical outcomes and quality improvement initiatives
  • Performs other duties associated with role as assigned

  • Key Qualifications :

  • Registered Nurse - Degree and CNO registration
  • Advanced Clinical Preparation with 5 years clinical experience in psychiatric nursing
  • CNA Mental Health Certification preferred and/or willingness to complete
  • Proven expertise in understanding and completing RAI-MH
  • Knowledge and experience in B Care Live and Test Fields
  • Completion of CIHI's RAI-MDS 2.0 Trainings preferred and/or willingness to complete
  • Experience in statistical analysis
  • Experience in information technology and its application in a clinical environment

    While this role is typically located at our Credit Valley Hospital site, it is currently being conducted remotely as part of the Flex First program.

    We offer employees, in roles that are conducive to remote work, the flexibility and the right mix of remote and in-office work (approximately 80/20). Acknowledging that the primary works space will be remote work from home, but there will be times when onsite presence is appropriate just as there will be times when onsite presence may not be possible.

    The expectation is that remote work locations, as part of the Flex First program, will be in the employee's place of residence, and adhere to all THP policies and procedures, with special consideration towards privacy. Since employees should be available to return to the workplace with 24 hours' notice, remote work outside of the province or country is not acceptable.
