This position reports to the Manager, Adult Intensive Services.Flexible Assertive Community Treatment (FACT) is a recovery-oriented, evidence-based best practice.

The target client population is those who require both assertive community treatment (ACT) with a larger majority of clients on the team who need less intensive treatment and support and could be served in a traditional intensive case management model.
To combine care for these two levels of care, the FACT team employs a flexible switching system where the intensity of services can be ramped up or ramped down as needed depending on individual client need.
