Starting Salary Range:
$47,500 - $52,000 annually, commensurate with experience

Position Status:
Full-Time Maternity Leave Contract with benefits and a complimentary YMCA membership

Nature & Scope:

The Case Manager works under the direction of the General Manager of Inclusion Services for Children.

They provide support to the School Age Child Care, Day Camp and Community Outreach departments through conducting program visits, assessing and creating support plans, building relationships and facilitating training sessions in the Hamilton, Burlington, and Brantford regions.

The Case Manager works collaboratively with both internal and external stakeholders to remove barriers to inclusion and support quality child care, day camp and outreach programs where children aged years feel connected, a sense of belonging, well-being, expression and engagement.


This is a maternity leave contract position; hours of work are based on a full-time 40 hours per week schedule, Monday to Friday, with the flexibility to be available early mornings and evenings to accommodate the needs of the Child Care Programs.

At certain times during the year, weekends may be required for the purpose of providing training. Use of a vehicle is required to visit YMCA programs within the community and provide on-site support when needed. This position is based in the Hamilton Region.


Responsibilities:


  • Empower educators to set up program spaces that are inclusive of all children by providing effective group resources, modifications, and adaptations.
  • Complete intakes and develop individual plans utilizing a strengthbased approach; outline effective strategies, as well as consider the health, safety, wellbeing, and engagement of children.
  • Complete program visits involving observations and the development of support plans for group management and/or individual children.
  • Utilize a variety of teaching techniques including relationship building, active listening, modeling strategies in action, providing practical strategies and onsite support.
  • Work collaboratively with Childcare Inclusion Services Team, School Age Child Care Area Managers, Child Care Managers and Day Camp Manager to ensure frontline staff are supported.
  • Maintain relationships with community partners by having knowledge of relevant services and resources.
  • Plan and organize schedule to support the needs of children and programs.
  • Communicate in a respectful manner, including listening and seeking to understanding different perspectives.
  • Collect, prepare, and maintain on going accurate information and documentation.
  • Consults with parent/caregivers, social workers, school staff, and other professionals, including involvement in parent/caregiver meetings.
  • Source, as well as create relevant and current resources to support inclusion.
  • Develop and lead professional learning sessions.

Qualifications:


  • Knowledge of child development demonstrated through at least 4 years of experience working with children with a variety of needs including developmental, physical, personal care, and socialemotional challenges.
  • Postsecondary education in a child related field such as Child and Youth Care, Counselling, Education, Social Work.
  • Certification in Crises Intervention and Prevention is an asset.
  • Maintains knowledge of current and emerging ministry directions and curriculum.
  • Ability to plan, facilitate and monitor strategies for children with moderate to severe special needs.
  • Excellent problemsolving skills with the ability to exercise sound judgement.
  • Strong organizational skills and ability to effectively prioritize responsibilities.
  • Ability to work effectively both independently and as part of a team.
  • Expert facilitation and conflict management skills.
  • Ability to work in a fastpaced environment.
  • Access to a vehicle is required for this position.
  • Current satisfactory Police Records Check with Vulnerable Sector Screening.
