The Opportunity:
The Nurse Practitioner will work collaboratively on the delivery of resident care to meet individual needs within his/her level of competency This role provides quality and safe resident care which helps to support and align resident needs What Is In It For You Collaborate liaise with MD DOC DOC team in care delivery Assess diagnose evaluate acute chronic illness changes resident conditions prescribe treatments diagnostics follow up care Support advocate for residents best interests engage resident / family / substitute decision maker (SDM) regarding Care Participate in resident care conferences Provide clinical leadership LTC team members centered around resident care needs including move in/out hospital discharge planning palliative end-of-life pain management dementia responsive behavior management Analyze identify interpret trends residence residence practice determine priorities educational programming team members residents families
