Description

Objective:
The Resource Supervisor will support and practice the mission, vision and philosophy of Satori Homes.

The Resource Supervisor is responsible for the support and supervision of designated foster homes, and the care and welfare of the children living in those home.

By providing direct support and supervision to foster families, children/youth, and support staff, the Resource Supervisor will ensure exceptional care and communication is maintained, and that necessary supports and services are available.


Children/Youth Related Responsibilities:


  • Arrange and facilitate Care Team and Plan of Care Review Meetings for the children and youth placed in the assigned homes.
  • Responsible for coordinating all people involved in the child/youth's care and bringing together professionals to discuss case planning and progress.
  • Act as the primary contact for Foster Care Providers, natural parents/guardians, placing worker, support workers, external service providers and children/youth.
  • Demonstrate an understanding of the effects that separation and loss, neglect, physical and sexual abuse, trauma, and mental health diagnosis can have on a child/youth. Coach and mentor Foster Care Providers on strategies to navigate these effects.
  • In collaboration with the Clinical and Program Lead, supervise and monitor delivery of therapeutic services to children and youth to ensure that all clinical services are delivered in accordance with the highest quality and standards.
  • Assist in the planning and coordination of family access.
  • Conduct inperson meetings with the Children / Youth monthly. Assess quality of service, areas of needed supports and appropriateness of placement.
  • Attend school meetings and maintain frequent contact to ensure success in school placement and advocate on the child's behalf.
  • May be required to attend medical and therapeutic appointments as needed.
  • Provide services in a manner which empowers children and youth and follows professionally recognized standards of ethical practice.
  • In collaboration with the Care Team, ensure all reports and documentation are completed as required
  • Facilitate placements of children/youth in assigned foster homes and ensure necessary paperwork is completed/obtained, and required supports are in place
  • Attend and participate in weekly Child in Care Team Meetings

Foster Family Related Responsibilities

  • Frequent contact with Foster Care Providers
  • Typically monthly, but no less than every 3 months to assess quality of service, areas of needed supports and appropriateness of placement.
  • Ensure Foster Care Providers are well informed of Care Plans, goals outlined in Plan of Care Reports, Safety Plans/ISP's, and permanency plans.
  • Provide support to Foster Care Providers in maintaining documentation and records (e.g MAR sheets, Fire Drills, Rights Reviews, Medication Plans for short term absences, Contact sheets, Home Visit/respite tracking etc )
  • Assist Foster Care Providers in arranging health care services.
  • Assist in the annual evaluations of all of the Foster Care Providers involved in providing services for the agency.
  • Serve as a liaison between the foster families and the different service agencies that children and youth are involved in.
  • Determine appropriate additional training that may be required to meet the families' needs and assist in maintaining an ongoing Learning Plan for Foster Care Providers.
  • Assist with arranging for respite and relief supports including foster family support worker schedules, respite weekend coverage and other coverage when required/requested.

Training and Development Responsibilities:


  • Assist with planning and organizing ongoing opportunities for Professional Development for both Foster Care Providers and Staff.
  • Maintain a presence to all agency members including Foster Care Providers, staff, volunteers and children and youth.
  • When applicable, review incident reports/critical incidents and in collaboration with Director of Service and Director of Clinical Services, determine the process and need for follow up.
  • Be willing to participate in professional development opportunities to increase knowledge base and stay innovative in practices and approaches.
  • Participate in rotating weekly On-Call (After hours Emergency) Schedule.

Administrative / Other Responsibilities:


  • Work with the Office Manager to Maintain Client Case Records and Foster Family Files both in paper and electronic copy (using the existing Data Management Program)
  • Maintain adequate case notes and prepare written reports including Plan of Cares; ensure all details are documented and reports are shared with appropriate people.
  • Maintain ongoing communication with Office Manager as needed.
  • Assist with day to day office tasks as required.
  • Maintains confidentiality in matters of agency operations, personnel, foster families, children/youth and birth families served.
