Your Opportunity:


The federally funded Connecting People & Community for Living Well work (April 2023 to March 2026) builds on the learnings of a January 2020 to March 2023 Health Canada grant.

The initial grant focused on supporting wellbeing at the community level, in particular the wellbeing of those living in the community with a diagnosis of dementia.


The 2023 to 2026 work creates the opportunity to: 1) spread the model to new rural communities; 2) scale the model to other underserved populations in rural communities; 3) sustain the model in the communities who participated in the grant; 4) share learnings from grant, and build a knowledge base to inform policy and infrastructure decisions to sustain this work; 5) understand usability of Competency Framework, evaluate impact of local activities, and complete an evaluative Social Return on Investment (SROI) analysis on the Connecting People & Community model.

The Practice Lead (PL) is a provincial out of scope position which reports to the Executive Director of the Strategic Clinical Networks.

Required Qualifications:

Undergraduate degree required. Where applicable, current registration with a professional association preferred. Applicants with equivalent combination of education and experience may be considered.


Additional Required Qualifications:

Minimum 5 years of recent experience in the health or social sectors. Front line experience and a broad knowledge of these systems is preferred. Planning and policy development experience is desirable. Demonstrated skills in leadership, including an ability to inspire and build confidence in others required. Strong facilitation skills preferred. Experience in problem solving preferred. Experience in working on a developmental, evolving project which involves ongoing exploration of new approaches and perspectives. Demonstrated interpersonal and written communication skills required, including an ability to relay or explain complex issues or difficult situations.


Preferred Qualifications:

Master's degree preferred. Demonstrated skills and experience in project management, change management, knowledge translation and quality improvement are preferred.
