Roles and Responsibilities


The primary responsibility of the Client Care Coordinator is to provide administrative support, program development, answer patient inquiries and book client appointments.


Care Coordinator Role

  • Conducting initial assessments of clients' situation to determine needs and goals
  • Establish open lines of communication for individuals with mental or emotional concerns
  • Take a detailed client history and note any health conditions
  • Treat patients with respect and dignity
  • Communicate information to patients regarding diagnosis and treatment goals
  • Maintain patient medical records (EMR)
  • Clinical assessments such as mental status exam and routine vitals for patients

Administrative Roles:


  • Participate in quality activities and continuous improvement initiatives
  • Develop and provide input on policies and procedures
  • Weekly and monthly reporting on site metrics and needs
  • Other Duties as assigned

Experience
French considered a strong asset

1+ years experience in the medical field or mental health considered a strong asset


Job Types:
Full-time, Part-time


Salary:
From $18.00 per hour

  • Secondary School (preferred)

Experience:

Medical Administration: 2 years (preferred)
