The
Nursing Supervisor / Memory Care Coordinator **is responsible for providing overall leadership and management of the 4th floor Memory Care area.

A leader in the day-to-day care and support of our reminiscent care floor residents and an outreach liaison to family members and substitute decision makers.

Develop a foundation for our 4th floor memory care program, including rolling out framework and training in conjunction with the Director of Care.

This role is a combination of RPN duties and Administration work.

Responsibilities:


  • Works closely with the Director of Care (DOC) to provide oversight of the reminiscent care floor
  • Schedules and coordinates ADL support; attends to the individualized care needs of the residents while demonstrating an indepth knowledge of the preferences and personalities of those who reside on the floor
  • Serves as a clinical leader and collaborates with other department managers and/or health care professionals to meet the complex care needs of the residents on floor.
  • Oversight of the medication administration program for residents on the reminiscent care floor
  • Provides ongoing communication to include: keeping family members and substitute decision makers informed of any acute care changes and/or behavior modifications; will make recommendations to improve on the quality of care and life enrichment programs available to their loved ones
  • Updates the DOC of any changes in condition and to make recommendations to the level of care and services being provided
  • Oversight of the groupbased and individualized life skills and enrichment programs being offered and modifies these experiences as required
  • Completes assessments and reassessments for all current and incoming residents as required under the RHRA. Wil update and audit Care Plans as needed to ensure all information is relevant and current
  • Will play an integral role in the hiring, training, education and development of the reminiscent care floor team; will monitor and amend the onboarding process to ensure it aligns with the expectations in service quality
  • Work closely with the DOC on annual reviews, recommended goals, strategies and support for performance improvement
  • Will provide budgetary insight to the DOC and assist in the overall annual forecasting of the Resident Care Department
  • Works monthly with an independent auditor to ensure all regulatory requirements are being completed in accordance with RHRA mandates and statutory requirements
  • Other duties and assignments as directed by the DOC

Skills/Qualifications:


  • RPN in good standing with the College of Nurses of Ontario
  • Maturity and good physical and mental health with a current TB screen, empathy, patience and tact are important qualities
  • Current and annual renewal of certification in CPR and First Aid
  • Good communication and leadership skills
  • Familiar with "Residents' Bill of Rights" as set by the RHRA
  • Familiarity with EMAR would be an asset

Physical Demands:


  • Considerable standing, walking, lifting and bending

Working Condition:


  • Shift work, including days, evenings, nights, weekends and holidays
