This is what you'll be doing:

The Admin Coordinator, Bundled Care/Integrated Care will provide administrative and general support to the Transitions and Integrated Care department, CCH OHT Clinical Programs reporting to the Director, Transitions & Integrated Care, as well as the Oakville Hospital Volunteer Association (OHVA) Board.

This role will assist in providing patient centered system navigation from hospital to community and ongoing community management, specializing in bundled care and transitions to care.

Will work closely with the Director, Transitions & Integrated Care to further integrate pathways that allow for wrap around care for our patients in the hospital, in the community and at home.

Working collaboratively with an inter-disciplinary team, both internal and external to Halton Healthcare, which includes but are not limited to Home Care Agencies, Peer Hospitals, Ontario Regional Health Teams, Community Physiotherapy Clinics, and the Ministry of Health.


The Admin Coordinator, Bundled Care/Integrated Care is a key resource and link for peer hospitals and community physiotherapy clinics regarding invoicing and payment processing for bundled care patients.

The Admin Coordinator, Bundled Care/Integrated Care will act as a liaison between these external care providers, receiving direct vendor calls and correspondence to clarify and review invoices and standards of care statistics for patient care services.

This position will review incoming invoices for accuracy, reconcile against patient records, track and record invoices and then move invoice forward for payment processing.


The Admin Coordinator, Bundled Care/Integrated Care will compile necessary data from Meditech and Practice Perfect, and will record the analytics that will be used further in invoicing and reporting.

The Admin Coordinator, Bundled Care/Integrated Care will be responsible for leading the invoicing processes for the Bundled Care Program and work to improve the efficiency of these process.

This will involve working closely with the Bundled Care Integrated Care Coordinator (BCICC) and other peer hospitals, community providers and regional health authorities to streamline processes.


The Admin Coordinator, Bundled Care/Integrated Care will utilize the organized data to operationally report NACRS and CIHI data to ensure efficient and seamless reporting.

This information is used to measure the key elements and variables of ambulatory care, which can improve the overall quality, efficiency and effectiveness of service.

Also allowing for comparison among peers regarding timely performance monitoring as requested by the Ministry of Health.

  • Undergraduate Degree in Business or related field of study
  • Two (2) years of recent related experience in a hospital setting and experience in working directly with internal and external community and regional health partners
  • Comprehensive knowledge of community resources and health care delivery systems in acute and community settings
  • Proficient Microsoft Office skills including Word, Excel, Access, PowerPoint and Outlook
  • Meditech experience preferred
  • Knowledgeable in NACRS Clinic Lite, CIHI, Practice Perfect programs
  • Master a high degree of multitasking while managing demanding priorities with elevated pressures of competing deadlines
  • Excellent interpersonal, communication, organizational and decisionmakin
