**Case Coordinator Position Overview:**

This role involves providing clients with comprehensive assessments to ensure a high-quality client experience.

The Case Coordinator is responsible for developing, in consultation with the client and caregiver, a comprehensive care plan to meet the client's goals.

Key responsibilities include maintaining competency, completing client assessments, developing therapeutic rapport, and utilizing technology in the home to complete comprehensive assessments.

Additionally, this role requires identifying and prioritizing environmental risk for client's health and safety, developing a comprehensive care plan, facilitating access to client-identified services, and making referrals to community and health resources as required.

The ideal candidate will possess strong attention to detail and accuracy, skilled communication and relationship management skills, and the ability to effectively manage multiple tasks while prioritizing, meeting deadlines, and working well under pressure.
