The Prince George Division of Family Practice Co-op (PGDoFP/the Division) is a local innovation in health care, and part of a province-wide initiative designed to strengthen primary health care in BC.

The Division coordinates local processes related to the overarching goals of developing capacity while supporting Family Physicians in the rapidly changing environment of Primary Care.

***The Primary Care Network (PCN) Manager role is collaborative between the Prince George Division of Family Practice, the PCN Steering Committee and the British Columbia Ministry of Health (MOH).


The Prince George Division of Family Practice aims to increase primary health care capacity and improve patient and population health outcomes by developing and implementing programs and services that will positively impact family physicians and their patients.


A Primary Care Network (PCN) is a clinical network of local primary care service providers located in a geographical area.

A PCN is enabled by a partnership between Divisions of Family Practice and Health Authorities.


The Primary Care Network (PCN) Manager plays a pivotal role in overseeing the development of the British Columbia Ministry of Health's Primary Care Network Initiative.


The Primary Care Network (PCN) Manager is the conduit between the Prince George Division of Family Practice, the Steering Committee, and the BC Ministry of Health (MOH).

The role has a cooperative reporting structure whereby they are employed by the Prince George Division of Family Practice but directed by the PCN Steering Committee.


Thus, the PCN Manager works with an interdisciplinary team, in a collaborative practice model, to operationalize the strategic priorities established by the PCN Steering Committee, to whom they are accountable.

DUTIES & RESPONSIBILITIES
  • Division, Steering Committee, and Ministry of Health_
This position is primarily responsible, but not limited, to the following:

Program Leadership, Project & Operational Management

  • Provides overall project leadership for the implementation of the Primary Care initiative within the Prince George PCN, which includes working with PMH programs/services, physicians, primary care providers, the health authority, community agencies, and other stakeholders.
  • Takes accountability for ensuring the Ministry of Health deliverables are communicated to stakeholders.
  • Engages in development of operational procedures/resources as required to ensure consistent practice across regional PCN(s)
  • Gathers data to fulfill Ministry of Health reporting requirements
  • Collaborates with Health Authority partners to submit required reports accurately and in a timely manner.
  • Promotes engagement of primary care providers in the PCN
  • Participates in identifying opportunities for improvement in processes to facilitate access and improve quality of care for populations served by the PCN.
  • Develops communication strategies to inform providers of resources available within the PCN and to inform the public about the PCN
  • Ensures that the oversight, implementation, and evaluation of the Primary Care Network is in alignment with Prince George Division of Family Practice strategic direction, vision, mission and values
  • Partners with the Primary Care Network Physician Lead during implementation and evaluation of the Primary Care Network to promote effective communication of Primary Care priorities
  • Identifies opportunities for improvement to existing processes, facilitates access to care, and promotes improvement to quality of care within the community
  • Provides project management and proposal development for the Steering Committee
  • Implements governance structures and operational plans.
  • Assists in the selection and monitoring of performance indicators at the local level. Prepares reports in collaboration with partners, as requested.

Partnerships and Engagement

  • Builds and maintains relationships with Northern Health, First Nations, and other community stakeholders.
  • Collaborates with Divisions of Family Practice and other organizations to facilitate recruitment and engagement of physicians and community members to participate in the Primary Care Network
  • Works with Primary Care Network stakeholders and community partners to evaluate where gaps in health care are present within our community
  • Pays particular attention to inclusion of local Indigenous groups and organizations in development of relevant programs

Leadership Responsibilities

  • Supervises the Primary Care Network Coordinator in accordance with the Division of Family Practice's programs and policies.
  • Engages in leadership duties such as interviewing, hiring, and training employees; planning, assigning, and directing work; participating in timely and effective performance management processes; addressing complaints and resolving problems.
