- Monitors and evaluates the nutritional status of each resident, including their current nutritional risk, prioritizing as required; documents these reviews as per MOHLTC standards and regulations.
- Assesses and documents the nutritional status of each new resident within MOHLTC standards and regulations.
- Prepares a nutritional care plan for each resident according to individual resident needs, which includes long-range goals.
- Works collaboratively as one of the clinical experts of the health care team relating medical and biochemical aspects of health and nutrition.
- Consults with and counsels residents and /or their families about specific nutritional needs.
- Provides direction to the Manager Food & Nutrition Services and the multidisciplinary team in order to meet the nutritional needs of residents, including regular menu review and approval.
- Provides direction to Pioneer Ridge Administration regarding nutrition and food service policies that reflect current standards of practice.
- Performs such other related duties as may be assigned.
- Degree majoring in human nutrition and dietetics
- Three years of therapeutic experience in a long-term care facility
- Current membership as a Registered Dietitian in the College of Dietitians of Ontario
- An understanding of the nutritional and eating needs of the frail elderly is necessary
- Ability to communicate effectively and concisely, both orally and in writing
- Proven ability to establish and maintain good working relations with private and public officials, superiors, co-workers, residents, families, and the general public
- Ability to work independently and as a team member
- Strong project/time management, analytical/critical thinking, problem-solving, and decision-making skills
- Ability to identify, understand, and meet/exceed the requirements of internal and external customers
- Must be at a basic skill level in word processing and spreadsheet software
- Must be competent within the meaning of the Occupational Health & Safety Act
- Must undergo a successful Police Vulnerable Sector Check (PVSC)
- Must provide proof of full COVID-19 vaccination at point of conditional offer
- Prior to commencing employment, will be required to review and attest/affirm to all Pioneer Ridge Staff Immunization Policies
