POSITION:

Complex Support Coordinator - General Focus


HOURS OF WORK:

Full-time contract (35 hours per week) ending August 8, 2025, with possibility of extension

Flexible work hours (early mornings, evenings, and weekends) and work location/environment


LOCATION:

Site flexible (Northern Ontario) - North Bay, Sturgeon Falls, Sundridge, Bracebridge, Parry Sound, or Timmins, ON (hybrid or primarily remote work may be considered)


HOURLY RATE:

Minimum $31.87 to maximum $38.78 per hour; Salary commensurate with education and experience

Our professional team helps more than 11,000 individuals in North Bay, Nipissing, Muskoka, Parry Sound, Timmins, and Northern Ontario each year.

We provide quality clinical, navigation and professional support in the areas of autism, behavioural, developmental, mental health, and rehabilitative services.

***: The Complex Support Coordinator (CSC) will provide coordination of the intensive supports needed for adults with developmental disabilities who have complex multisystem needs. These adults require a high level of active engagement and ongoing mentoring. The CSC will be responsible to provide a variety of supports including conducting assessments, coordinating the development of complex support plans, establishing linkages to services and coordinating cross-sector support for case resolution with the goal of improving the client's access to supports and services, using an evidence-based, person-centered approach.


QUALIFICATIONS:


  • Four (4) year Bachelor degree in Social Work (preferred), Psychology, Human Development, or Social Services
  • Registered Social Worker with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) preferred
  • A minimum two (2) years of related experience is required
  • Experience in the developmental disability sector is preferred
  • Aware of interagency and other resources available to clients is an asset
  • Confident chairing meetings and making presentations
  • Strong organization, communication, and problemsolving skills
  • Excellent software skills using the Microsoft Office suite (Word, Excel, PowerPoint, Teams, SharePoint, and Outlook)
  • Demonstrated commitment to workplace health and safety, with a willingness to contribute and lead in the culture of safety and prevention of adverse health events for clients and their families/caregivers, colleagues, other community members and yourself
  • Demonstrate ability and commitment to maintain confidentiality
  • Successful applicants will be required to provide:
  • proof of eligibility to work in Canada
- a valid Ontario driver's licence and use of a reliable motor vehicle; insurability on Agency vehicle insurance policy
- clear Criminal Record Check, including Vulnerable Sector Search
