About the Opportunity:


Under the supervision of the Provincial Manager Intake & NOAT, the Intake Coordinator provides initial contact for Single Entry Access to Continuing Care Services.

Using a computerized Intake tool, the Intake Coordinator receives and processes new and or existing client referrals, determines service streams, either Home Care/LTC or Adult Protection and assigns to a Team, Care Coordinator or Adult Protection worker.

The Intake Coordinator provides information on the Continuing Care program/services and the criteria required for program access.

Referrals and inquiries come from the public, client/family members, health care facilities, Department of Community Services, community physicians and police to name a few.


Program referrals are for Nursing (acute and chronic), Home Support (personal care, light housekeeping, meal preparation), Long Term Care Placement (including out of province), Respite Care, Palliative Care, Home Oxygen, Caregiver Benefit, Self-Managed Care, Equipment and Bed Loan Program, Quick Response Program, Supportive Care Program, Personal Alert Assistance Program and Seniors Wheelchair Program.

Service Referrals are for Adult Protection and Protection of Persons in Care Act.


About You:

We would love to hear from you if you have the following:

  • Grade 12 plus completion of a Secretarial/Office Administration program
  • Two years of related experience in an office environment required
  • Previous work experience in call centres an asset
  • Experience on use of the complete SEAscape (Momentum Healthcarecomputerized referral tool)
  • Experience with Horizon Patient Folder/clinical portal/Meditech
  • Excellent Customer skills
  • Ability to handle large volume of calls and faxes and conflicting priorities
  • Excellent Organizational skills
  • Competencies in other languages an asset, French preferred

Hours:


  • Long Assignment Fulltime Position; 70 hours biweekly
  • Approximately 12 months
  • Shifts may include days, evenings, weekends and holidays

Compensation and Incentives:

$ $24.31 Hourly
