Job Summary:


Participate as a member of the Adult Day Program team, providing personal care support to clients attending the day program.

This includes supporting the client with activities of daily living, restorative programs, health and wellness promotion and assisting with recreation programs.


Duties & Responsibilities:


  • Provide personal care for clients of the Adult Day Program.
  • Serve client meals and is responsible for assisting with meals and monitoring client's food consumption.
  • Assist with recreation and therapeutic programs including one to one activities
  • Observe reports and record changes in the client's status and contributes to the development of client care plans.
  • Assist with housekeeping duties as assigned such as cleanup of program areas and meal area.
  • Communicate effectively with clients, staff and families.
  • Provide excellent client service at all times.
  • Work cooperatively with team members.
  • Participate in quality improvement activities as directed.
  • Perform other duties as assigned.

Skills & Qualifications:


Essential

  • Personal support worker certificate from a recognized college.
  • Previous experience in geriatrics, rehabilitation or community setting such as an adult day program.
  • Experience working with those with dementia, responsive behaviors and adults with disabilities

Preferred

  • Understanding of the Client's Bill of Rights as per the Ministry of Long-Term Care.
  • Familiarity with computers including MS Office and Point Click Care.
  • Additional training including Food Handlers Certificate, Gentle Persuasive Approach (GPA), Montessori Approaches for Dementia and First Aid certification.

Working/ Employment Conditions:


Working Condition

  • Physical demands include lifting, pushing and pulling.
  • Flexibility in work schedule for occasional evening and weekend work as needed.
  • Travel will be required, incumbent must provide their own transportation.

Employment Condition

  • Current (obtained within the past six (6) months), original and acceptable Criminal Records Check, including Vulnerable Sector Screening by the first day of employment.
  • Current twostep tuberculosis test (obtained within the past six (6) months) by first day of employment.
  • In accordance with requirements in Ontario Regulation 191/1
