Your Opportunity:


Reporting to the Manager of NW campus, the Registered Social worker II (RSW II) works independently as a member of a multi-disciplinary team on FMC unit 24, providing evidence-informed and best practices in assessment, case management, crisis intervention, education and referral sources for clients, their families as appropriate and follow up to mental health patients.

The RSW II supports clients as they move into and out of Outpatient Clinics.

The RSW II provides a range of generalist social work interventions with clients (individuals, families and groups, populations, organizations and communities).

As an RSW-SW II, you will respond to the instrumental, emotional, and social needs of clients and families using a variety of psychosocial interventions including counselling and psychoeducation.

An essential part of the role is to identify and address/respond to structural barriers, social justice and health equity issues that impact the social determinants of health of individual clients and families.

The RSW-SW II works with clients to achieve recovery oriented goals and social wellbeing.

The RSW II collaborates with inter-professional health team members, public sector, and community agencies to advance care for clients; and participates in quality improvement initiatives.

The RSW-SW II contributes to their professional community; participates in peer mentoring and supervises social work students and may provide supervision to Registered Social Worker SW I, and Social Workers on the provisional register, as requested.

The frequency of these activities will vary depending upon the program needs.

Required Qualifications:

Experience in health care or settings with transferable experiences is preferred, e.g. (Human Service Agencies, Primary Care Networks, etc.).
