About the Opportunity:


The Nova Scotia Brotherhood and Nova Scotia Sisterhood is a community based primary health care initiative serving the communities of North Preston, East Preston, Cherry Brook and Lake Loon, Dartmouth North and North-End Halifax.

Beechville, Hammonds Plains, and Lucasville and Three Mile Plains that focuses on the provision of primary medical care and programs and services to support health and wellness.

The Nova Scotia Brotherhood and Nova Scotia Sisterhood works in collaboration with other provider groups, community partners and services to offer programming and supports to the four communities.


The Nova Scotia Brotherhood and Sisterhood is a small team that works to promote health within HRM and to assist with physical and psychological health.

Providing support in the areas of medical, community outreach and system navigation, the team works with the client around their social determinants of health in a holistic approach.


Reporting to the Health Services Manager, the Team Leader demonstrates flexibility and innovation in supporting citizens in the communities by accessing the services, programs, and/or resources that can support health enhancing behaviors.

The Team Leader provides guidance and support for clients, including providing information about appropriate supports and programs that will meet health and wellness needs and social supports that can assist with daily living.


The Team Leader will be responsible for the development, implementation and coordination of The Nova Scotia Brotherhood and Sisterhood programs and services in collaboration with community partners and other system stakeholders.

These integrated services will encompass health promotion, disease prevention, chronic disease management, and population health. This individual acts as a role model and exhibits team building and leadership skills.

The team Leader will demonstrate effective communication and organizational skills and works with the primary medical care and broader health care team, community groups and agencies, other providers, and partners to ensure consistent, equitable, timely and coordinated care is provided to clients.

The incumbent is a member of an inter-professional team and collaborates with colleagues to determine team goals and objectives, participates in the evaluation of team effectiveness, solves problems, and shares responsibilities.

Excellent interpersonal and written communication skills are required.


About You:

We would love to hear from you if you have the following:

  • Baccalaureate degree in in a health related discipline (i.e. Physiotherapy, Dietitian, Social Work, Occupational Therapy, Nursing)
  • Appropriate licensure based on discipline
  • Three years' clinical experience required (with clinical experience in the last year working with individual and/or group patient education)
  • Valid driver's license and access to a reliable car for travel for work
  • Previous experience in team development and leadership working within a collaborative interdisciplinary practice
  • Experience working in a Primary Health Care setting/community based team is required
  • Experience working with diverse communities is preferred
  • Clear understanding of the following; primary health care and Black health issues
  • Experience using an Electronic Medical Record
  • Accuro is an asset
  • Values diversity and team work
  • Demonstrated skills in partnership development and stakeholder collaboration
  • Understanding of the unique and historic barriers to accessing healthcare for these communities
  • Appreciation of providing services through an Afrocentric lens
  • Individual and group education/facilitation skills utilizing adult education, selfmanagement support and/or motivational enhancement principles
  • Experience in community development and working with people living with a chronic condition is preferred
