The Urban Health and Homelessness Services team provides resources, counseling, and referrals to individuals who are low-income, socially isolated, homeless or hard-to-house. We specialize in trauma-informed mental health and harm reduction services.

This position offers mobile eviction prevention support to vulnerable households across greater Toronto as referred by the City's EPIC Intake Team. You will assess individual needs, provide referrals, create personalized action plans using a Housing First approach. Your role is critical in coordinating landlord-tenant mediation for successful outcomes while educating on their respective rights and responsibilities.
