ROLE PURPOSE

This position will provide casework and behaviour support to residents of Connective Support Society (Connective)'s programs in order to enhance their quality of life, to ensure the orderly and safe operation of the facility, and to maintain the goals of the treatment plan.


STATUS AND CLASSIFICATION
This is a temporary position until return of incumbent.
Unionized; this position requires union membership. It is open to all genders.

HOURS (22.5 HOURS PER WEEK)
Wednesday to Friday,


KEY ACCOUNTABILITIES
Programs

  • Interviews clients to prepare care plans and background information for which confidentiality is required
  • Under the general guidance of the Program Manager, assumes a primary or key worker role with assigned resident(s)
  • Provides input to the Program Manager and/or other staff with regard to the development of appropriate care plans to achieve the resident's objectives
  • Is cognisant of, and work towards the implementation of the care plan established for each resident
  • Contributes to the updating and implementing of personcentered safety plan and behavior supports
  • When applicable maintain 24 hours supervision of the resident in accordance with the safety plan
  • Develops interpersonal therapeutic relationships with psychiatric patients
  • Connects clients with health and psychiatric services
  • Liaises with treatment team to observe and report client adherence to their legal conditions
  • Participates in clientcentered goal setting, taking into consideration the input from treatment team
  • Meets with each resident on a routine basis to discuss progress and plan future interventions
  • Assists the Program Manager and/or other staff in the identification of potential problems and reports any difficulties
  • When assigned, participates in social, recreational, employment, educational, and other programs as appropriate to assist the resident's case/care plan
  • Contributes to the evaluation of the resident's progress and makes both written and oral reports to be available to the remainder of the staff team
  • Maintains client files including statement of goals, case/care plans, summaries, case notes, reviews and closing reports
  • Develops and maintains an inhouse resource area containing current information/brochures of programs and services which are available to residents
  • Ensures residents are in compliance with the terms of their residency and are aware of the rules of the residence
  • Ensures residents are in compliance with the terms of their release conditions
  • Acts as an onsite advocate for each resident
  • Assists residents with activities of daily living
  • Provides emotional support and feedback to residents and their families
  • As appropriate, supports residents towards integration and normal functioning within the community
  • Acts as a positive role model for residents, providing life skills training and behaviour management as appropriate
  • Dispenses and ensures the taking of medications as required
  • As necessary, provides transportation and assists residents to attend appointments
  • Staff develop and deliver inhouse programs to address the needs of the residents
  • Demonstrates exercises and supports the residents with their exercise routines.
  • Participates in physical fitness daily with the resident.
Participates in various physical activities including, but not limited to:
hikes, the Coquitlam Crunch, interval running, long walks, basketball, soccer, swimming, cardio circuits, treadmill, and cardio machines.

  • Adhere to the residents' fitness requirements as outlined in the behaviour plan.
Community

  • Identifies social, economic, recreational, employment, and educational services in the community that will meet the needs of the resident
  • Collaborates with a multidisciplinary team and consults with team psychologist and/or psychiatrists, behaviour consultants, and forensics to develop, implement, maintain, and adjust individualized treatment plans
  • Maintains effective liaison with other agencies, professionals, government officials, families, and the community in order to contribute to the success of the case/care plan of the resident
Residential

  • Supervises the activities of the residence, its residents, and volunteers while on shift including maintaining case notes and the logbook
  • Along with the other program staff, monitors and ensures effective operation of the program through identifying any areas of concern and notifying the appropriate parties
  • Assist the resident with meal scheduling and preparation, including food inventory.
  • Ensures the security of the residence
  • Is responsible for assisting the resident with housekeeping and grocery shopping
