About the Opportunity:

The Healthcare Social Worker (Masters) is a core member of the community-based Early Supported Stroke Discharge Team. This position reports to the Program Manager - ABI and is part of an inter-professional team.

Healthcare Social Workers on the team, work in close collaboration with other team members to provide a diverse range of interventions to patients and their families.

The goals of the Early Supported Stroke Discharge Team are to provide care for recently discharged, community dwelling patients who require ongoing stroke rehabilitation in order to remain living in their community.


The Social Worker works in close collaboration with other team members to provide a diverse range of interventions to clients and their families.

The goal is to enable clients to achieve their optimal level of functioning and health.

Social Workers on the team provide a diverse range of individual and group-based interventions to clients and their families enabling them to achieve their optimal level of functioning and health.


These interventions include:
psychosocial assessments, adjustment therapy, supportive therapy, crisis counseling, resource counseling, discharge planning and case management.

The Health Care Social Worker is actively involved in advocacy, education, program planning, consultation and collaboration on behalf of individuals/groups within the institution and community.

The role is multifaceted and dynamic requiring the Health Care Social Worker to work autonomously as well as collaboratively.

This position may be required to provide service throughout the Rehabilitation and Supportive Care portfolio.


About You:

We would love to hear from you if you have the following:

  • Master's Degree in Social Work from a recognized university
  • Registered or eligible for registration with the NSCSW
  • Two (2) years of clinical experience as a clinical Social Worker required
  • Knowledge of the fundamentals of human behaviour including systems theory, family dynamics, group dynamics, and theories of behaviour change
  • Awareness of local community resources and agencies would be an asset Valid Nova Scotia Driver's License required and access to a reliable automobile
  • Maintains current certification in CPR
  • Effective communication and interpersonal skills
  • Training or experience in motivational interviewing and client centered treatment approaches are strong assets
  • Training or experience in groupbased treatment approaches, neurosciences and acquired brain injury rehabilitation are strong assets
  • Completion of training in advanced nonviolent crisis intervention, and suicide/violent risk assessment required or willingness to complete same Qualifications continued
  • Willingness to participate in the development of the Early Supported Stroke Discharge Team and communitybased programs
  • Willingness to work within a collaborative interprofessional context
  • Willingness and ability to travel and see patients potentially within a variety of settings within the HRM catchment area
  • Willingness to use personal vehicle for work purposes and to inform insurer of same
  • Competencies in other languages an asset, French preferred

  • Permanent, Parttime 50% position; 37.5 hours biweekly

Compensation and Incentives:

$ $48.10 Hourly
