QUALIFICATIONS:

  • Graduation from a recognized program for Licensed Practical Nurses
  • Current practice registration with CLPNBC without restrictions.
  • Previous experience in a residential care setting preferred.
  • SUMMARY OF JOB RESPONSIBILITIES:
  • Under the direction of the Director of Resident Care/General Manager/Resident Care Manager, the LPN coordinates the delivery of care to the Residents in accordance with the established standards, policies and procedures as set by Trillium Communities, the scope of practice of CLPNBC and other legislated scopes of practice.
  • DUTIES:
  • LPN's work to their full scope of practice and this includes but may not be limited to:
  • The receiving and transcription of physician's orders using a prescribed tool such as SBAR.
  • Ensures that orders are correctly processed, recorded and acted on.
  • Medication Administration to the full scope of practice as stated by the CLPNBC.
  • Provides emotional support and comfort to clients and their family. The pronouncement of expected death as per the LPN scope of practice as set forth by CLPNBC.
  • LPN's will be responsible for coordinating and supervising others who will deliver direct care, such as the Care Aides.
  • Provides personal care such as assisting clients with bathing, dressing, care of skin and hair, assists with changing beds, toileting and ensuring client exercise programs are carried out as directed.
  • Documents client care and progress as per CLPNBC standards of documentation
  • Participates as a member of the interdisciplinary team by assisting in the development, implementation, assessment and evaluation of client care plans.
  • Making or adjusting resident assignments as required.
  • Assists in maintaining a physical and psychosocial environment which meets the evolving care of the Residents.
  • Ensures the proper use and care of equipment used in providing care to the Residents and promptly reports any malfunctions to the DOC/GM/RCM.
  • Assists in maintaining adequate quantities of nursing care supplies on the floor.
  • Reports to the DOC/GM/RCM or designate all Incidents as outlined in the Community Care Facilities Licensing directive and completes the appropriate paperwork.
  • To assist in the orientation of new staff and the mentorship of students.
  • To perform other related duties as assigned by the DOC/GM/RCM.
  • Reads and follows Trillium Community policies and procedures.
