Vancouver Coastal Health is looking for a Rehabilitation Assistant to join the Personalized Support and Stabilization team working out of the Three Bridges and Robert and Lily Lee Family Community Health Centres in Vancouver, BC.

As a Rehabilitation Assistant you will

Function under the operational supervision of the Manager and the work direction of a licensed designated occupational therapist or physiotherapist to work as a member of the health care team.

Provide assigned physical / occupational therapy therapeutic interventions.

Assist in modification of Occupational Therapy/Physiotherapy care plans and facilitate transition to community programs in order to enable clients to maximize potential for independence.

Provide functional direction to family, and report relevant observations of client's condition in writing or by utilizing basic computer literacy.

Manage equipment safety and inventory.

ABOUT PERSONALIZED SUPPORT AND STABILIZATION (PSS)

The Personalized Support and Stabilization (PSS) team is a short-term community-based team of interdisciplinary professionals who work collaboratively, using a holistic approach to facilitate early discharge and support clients' transition back into the community.

The team focuses on helping clients regain as much independence as possible in their own homes.

Education & Experience

Completion of a recognized Diploma level Rehabilitation Assistant Program with Occupational and Physical therapy components plus one (1) year's recent, related experience working with target population.

Valid BC Drivers License. Local area travel requires the use of a personal vehicle for which mileage will be reimbursed.

Knowledge & Abilities

Ability to communicate effectively both verbally and in writing.
Computer literacy skills.
Ability to work collaboratively with others.
Physical ability to carry out the duties of the position.
Ability to organize and prioritize work.
Ability to observe and recognize changes in clients.
Ability to analyze and solve problems.
Ability to establish and maintain rapport with clients.
Ability to motivate clients and to encourage progression through therapeutic goals.
Demonstrated ability to provide self-care instruction and education.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.

Successful applicants may be required to complete a Criminal Records Review Check.

As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections.

Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks.

For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
