Edmonton Integrated Services (EIS) is currently seeking an individual to fill the
temporary position of Disability Service Practitioner (CDSP) in our Community Support Program.

Position Summary:

The Community Disability Practitioner is a front-line community disability services role that is responsible for the provision of day-to-day, hands-on support to individuals with developmental disabilities and behaviors of concern.


Duties and Responsibilities:


  • Under the direction of the Program Supervisor or Program Coordinator, the Community Disability Practitioner is responsible for the ongoing, daytoday operations of our program
  • Establish daily routines for the home(s) that reflect the overall needs of the individuals
  • Establish and maintain appropriate data collection and reporting procedures that are in line with the policies and procedures of the Agency
  • Ensure that Support Plans are monitored and maintained monthly and that strategies listed in the plan are being documented and employed by all team members
  • Establish and maintain appropriate communication channels within the program and with external supports
  • Ensure administrative procedures are established and maintained

Hours:
Full and part time shifts available


Formal Education:
Diploma/Degree in Rehabilitation or Human Services Studies.


Experience:
A minimum of one (1) year experience in providing support to adults with mental/ developmental disabilities. Experience with mental health issues is an asset.


Knowledge, Skills, Abilities:


  • Ability to confidently provide frontline, face to face support with individuals with varying challenges
  • Excellent verbal and written communication skills
  • Basic to intermediate computer skills
  • Strong time management and organizational skills
  • Requires the ability to work independently
  • Flexibility to sliding hours
  • Must be able to identify issues, set priorities and implement an appropriate plan of action
  • Proven ability to work collaboratively

Other:


  • Must have a reliable car with personal liability insurance with a minimum coverage of two million ($2,000,000.00)
