Home and Community Care Support Services Champlain is one of 14 Home and Community Care Support Services organizations in Ontario. We are dedicated to ensuring our more than 64,000 patients annually receive the highest-quality health care and services at home. Our patient-care coordination teams help develop care plans for people of most ages, focused on maintaining each patient's independence and dignity. We also provide long-term care home placement services, and facilitate our patients' access to the community-based services they need.


Position Summary:


The Contract Specialist is responsible for supporting the development, implementation and monitoring of Service Provider Organization (SPO) and Vendor contracts and performance management.


Primary Responsibilities:

Working in a small team, the Contract Specialist coordinates the end to end contract process with assigned contract SPOs and Vendors by:

  • Responsible for the development, distribution and monitoring of all SPO and Vendors contracts including Specialty Contracts such as Hospice, Private and Home School and Family Managed Health Care (FMHC) contracts;
  • Maintains contract related documentation and databases to ensure contract renewals, insurances, and financial statements are up to date in advance of renewal;
  • Monitors SPO performance within the context of provincial performance goals defined by the Ministry ensuring service delivery standards and contractual obligations are being met;
  • Utilizes a provincial portal and other internal reports, retrieves data and prepares quarterly performance summaries and dashboards for each contracted SPO and Vendor;
  • Coordinates and supports quarterly performance meetings with contracted SPOs and Vendors, elevating any major noncompliance issues to the Manager and/or Director and ensuring any followup items are addressed appropriately;
  • Completes a quarterly annual summary of SPO performance;
  • Review, analyse and trending of missed care events, taking necessary actions with service providers to mitigate risk to the patient and the organization;
  • Collaborates with Patient Services and Quality regarding SPO/Vendor performance monitoring and improvement opportunities;
  • Proposes strategies and coordinates approved problem resolution activities regarding SPO/Vendor contract issues;
  • Liaises with SPOs to secure services for patients with gapped care plans and manages waitlist releases in consultation with Patient Services,
  • Provides contract expertise and interpretation to staff, management, SPOs and Vendors to ensure contract compliance
  • Participates in ad hoc local and regional projects that facilitate organizational process improvement;
  • Maintains effective business relationships with SPOs and Vendors to promote quality service and resolve issues;
  • Liaises with stakeholders internally and externally to implement best practices within scope of position;
  • Supports the internal processes required to setup SPO contracts in the Client Health Record Information System (CHRIS) to reflect contractual rates, market share and performance metrics in collaboration with the Applications team;
  • Acts as the SPO/Vendor contact for health partner gateway (HPG);
  • Acts as the departmental contact for HPG;
  • Assists with the analytic analysis In the preparation of briefing notes; and
  • Performing other related duties as may be required on a less regular basis.

Education and Experience:


  • University degree in Health Sciences, Supply Chain Management, Health Administration, Health Planning and Business Administration or other relevant program or the equivalent combination of education, training and related experience.
  • Certified Supply Chain Management Professional (CSCMP) designation is an asset.
  • Project Management Professional (PMP) designation is an asset.
  • Bilingualism (French) is considered an asset.
  • 2 years' experience is a similar role.

Knowledge:


  • Knowledge of concepts, issues, leading practices and procedures for contract administration management, monitoring, and compliance
  • Proficiency in data collection, analysis and reporting to support quality monitoring and evaluation strategies
  • Demonstrates analytical, critical thinking, conflict resolution and decision making capabilities
  • Knowledge of HCCSS' mandate and its role within the health care system
  • Understanding of the Ontario health care system, service delivery models used in the community and the HCCSS' evolving role and mandate.
  • Advanced proficiency in Excel and other Microsoft Office Suite programs
  • Sound knowledge of CHRIS functionality
  • Selfmotivated with demonstrated ability to work effectively, accurately, work independently with mínimal supervision and take initiative within the job scope
  • Demonstrated ability to work in and respond to a dynamic and changing environment
  • Ability to handle sensitive and confidential information in a discreet and professional manner;
