JOB SUMMARY

Responsible to the Director of Care and working under the general supervision of a Registered or Licensed Practical Nurse; this position performs a variety of routine nursing procedures and related duties necessary in caring for the personal needs and comfort of residents.


TYPICAL DUTIES AND RESPONSIBILITIES

Demonstrates competency in day-to-day operations:

  • Can describe what the resident has stated is important in their care and outcomes and effectively communicated the resident goals with the team.
  • Utilizes knowledge and clinical expertise in identifying strengths and capabilities that assist and maximize independence for the resident.
  • Follows, participates in and implements interventions as per resident care plan to improve and maintain function to ensure safe work practices.
  • Monitors and evaluates effectiveness and reports to the care team.

Coordinates resident care delivery with a person-centered philosophy:

  • Ensures completion of assignments by meeting deadlines and commitments whilst respecting resident preferences, routines and variations.
  • Provides support to peers in prioritizing and planning care delivery
  • Identifies own limits and working needs and asks for assistance as required.
  • Follows all facility policies and procedures.
  • Able to evaluate and report residents response to treatments

Communication and Interpersonal skills:

  • Promotes a team approach and collaborative work environment
  • Uses a dementia sensitive approach and is intuitive and receptive to resident responses.
  • Reports important information to the nurse and communicates concerns effectively to the team.
  • Documents and records all pertinent information in the appropriate electronic record (POC).
  • Uses effectively and appropriately all communication tools care plan, ADL sheet, communication board and POC.

Teamwork:

  • Promotes and facilitates a safe environment for residents and staff
  • Promotes dementia sensitive and person centered philosophy of care
  • Is flexible and adaptable to care/team needs.
  • Respects diversity of all people
  • Able to negotiate and compromise as required.
  • Contributes to and supports team decisions.
  • Is respectful, polite and courteous when dealing with residents, staff and visitors.
Follows VIHA, Accreditation standards, and GVL policies and procedures.
Performs other duties as required


JOB SPECIFICATIONS

Education, Training and Experience:

  • Current practicing registration with the BC Care Aide & Community Health Worker Registry
  • Grade 10, plus graduation from a recognized Nursing Assistant Program or an equivalent combination of education, training and experience

SKILLS and ABILITIES
Ability to provide nursing care to residents and families in a professional manner consistent with policies, procedures, standards, and regulations in the following areas:

  • Assessment-cognitive, physical, functional, and psychological
  • Effective communication skills oral, written, and computerbased
  • Effective interpersonal skills
  • Ability to prioritize, organize, and coordinate your own work
  • Ability to communicate effectively both verbally and in writing.
  • Ability to deal with others effectively.
  • Physical ability to carry out the duties of the position.
  • Ability to organize work.
  • Ability to operate related equipment.
  • Computer competency
  • Demonstrated critical thinking and problemsolving skills
  • Ability to effectively employ human relations skills including conflict resolution
