Are you looking for an opportunity to play a key leadership role in a progressive forward thinking and growing community hospital located in Ontario's historic Huronia on the shores of beautiful Georgian Bay, just 90 minutes north of Toronto?


As the
Patient Flow Coordinator, you will be an integral part of the GBGH Team, reporting directly to the Director, Clinical Services.

You will be accountable for providing leadership and direction for the effective and efficient patient flow from GBGH care units to patients' discharge destinations, while providing support functions of primary nursing care, as well as overseeing the Patient Flow Navigators.


Job Requirements:

QUALIFICATION & EXPERIENCE

  • Bachelor's Degree or higher in Health Carerelated field, e.g., Nursing, Social Work
  • Training & experience with legislation & programs associated with Discharge Planning
  • Five (5) year's recent, related clinical including discharge planning and/or social work
  • Progressive leadership experience, including direct supervisory experience or an equivalent combination of education, training, and experience
  • Experience in managing interdisciplinary teams
  • Experience in engagement of community partners/stakeholders
  • Experience in facilitating, collaborating, and negotiating for effective resolution of human resource issues
  • Knowledge of legislative requirements, medicallegal issues, & a commitment to keeping abreast of changes
  • Proficient with Microsoft Word, Microsoft Excel, Microsoft Outlook
  • Knowledge of the broader health care system including the role/function of Ontario Health and provincial strategies and priorities
  • Ability to maintain composure under high pressure or stressful conditions while meeting multiple competing deadlines
  • Ability to create and foster an inclusive environment that achieves results by valuing, supporting, and rewarding collaboration and cooperative working relationships
  • Well organized and initiativetaking with the ability to maintain composure under high pressure or stressful conditions while effectively with multiple competing priorities

  • Strong ability to develop and foster collaborative working relationships to support the professional practice and ongoing development of staff
  • Ability to work with a diverse group of leaders and stakeholders with demonstrated success in moving key stakeholders to positive resolution and to facilitate and coach clinical areas on change initiatives
  • Demonstrated success in providing leadership in a way that facilitates effective teamwork, highly developed interpersonal and negotiation skills, and the ability to develop consensus with a variety of stakeholder objectives
  • Ability to promote development of clinical research opportunities to ensure evidencebased practice within an interdisciplinary teambased environment
  • Client serviceoriented, with a demonstrated commitment to valuing diversity and contributing to an inclusive working environment
  • Professional and respectful of protocol and confidentiality
  • As a partially designated French speaking organization, French verbal and written communication skills are considered an asset in this position

Georgian Bay General Hospital (GBGH) is a 113-bed hospital located in Midland, ON and serving a catchment area that includes the communities of Midland, Penetanguishene, Township of Tiny, Township of Tay, Christian Island, Georgian Bay Township and Springwater Township.

It serves a population base of 55,000 swelling to more than 150,000 for six months of the year with seasonal residents.
