Manager, integrated services and care (HATP), with dotted line reporting to

Director, Operations and Organizational Systems (FHC)


Location:
Health Access Thorncliffe Park (HATP) sites. This position will be located within any of the HATP service locations based on community and program requirements


Background:

Health Access Thorncliffe Park (HATP) is committed to providing coordinated Health & wellness services to the Thorncliffe Park community. It is an innovative partnership between Flemingdon

Health Centre (FHC) and TNO-The Neighbourhood Organization (TNO) funded by Ontario Health.


HATP is a community-driven initiative aimed to enhance interdisciplinary comprehensive primary health care, and improve access to health, social, and community services in Thorncliffe Park.

Position Summary:


HATP is seeking a Decision Support Specialist who will support data quality, management, strategic planning, reporting, risk management, and auditing requirements of the program.

This includes extracting data/ preparing queries, analyzing data, and preparing internal/external reporting to support decision making and quality improvement across HATP.


Responsibilities:


Strategic Decision Support:


  • Promote evidencebased planning to support management team on decision making process.
  • Provide strategic business intelligence based on internal and external data.
  • Lead collaborative processes to identify and address the program's information needs
(decision-making, reporting, quality improvement).


Data Analysis and Management:


  • Design, run, and update data queries based on program and reporting requirements.
  • Analyze and present data to support decisionmaking and internal/external reporting.
  • Support a dashboard of key indicators for HATPs KPI and the Thorncliffe Community
Hub, a multi-agency/multi-service hub.

  • Ensure efficient data extraction and reporting.
  • Offer expertise to the HATP team and other organizations on data management practices and tools.
  • Collaborate with HATP partners on data accuracy, error detection, and correction methods.
  • Establish and enforce data management policies and procedures.
  • Design procedures to support privacy and security, including a data/user auditing system.

Training and Technical Support:


  • Offer expertise and advice on data management practices to diverse groups.
  • Train staff on data quality, data analysis, and reporting procedures.
  • Support users in the Electronic Medical Records system (PS Suite) and troubleshoot related issues.

Be the primary point of contact for EMR support and other data-related tools.

Quality Improvement:


  • Lead and support Quality Improvement Initiatives (QI)
  • Develop measures to ensure team effectiveness in maintaining service quality and relevance.
  • Report on program outcomes and provide data to gauge performance against objectives.
  • Offer leadership in creating measures of client care quality and satisfaction.

System & Project Management:


  • Facilitate planning activities for new site additions to the program.
  • Participate in system changes to ensure compliance with data standards.
  • Act as a primary contact for Alliance for Healthier Communities.
  • Oversee the migration to new EMR systems when required.

Compliance & Governance:


  • Act as Local Registration Authority for ONE ID Business Support eHealth Ontario.
  • Serve as Privacy Support Officer.
  • Ensure work aligns with safety legislation and the organization's antioppression and cultural competency principles.

General Duties:


  • Conduct works safely and in line with organizational values.
  • Handle other duties and responsibilities as assigned.

Skills and Qualifications:


  • Undergraduate degree/diploma in Computer Science, Health Administration, Medical
Informatics or related field

  • Minimum of 3 to 5 years' experience in data management, preferably in a health setting
  • Knowledge of word processing, spreadsheet, database, and statistical programs as well as various EMR systems
  • Current utilizations include Practice Solutions Suite
  • Experience and working knowledge with health, settlement, and social services sector databases (municipal, provincial, or national level) is considered an asset.
  • An understanding of clinical, statistical, and financial information and how it influences healthcare funding models and impacts patient care
  • Knowledge of health information and data management within the health sector, including various systems and approaches for extracting, analyzing and presenting information for use in internal/ external reporting, decision manag
