As a Care Coordinator, your main tasks will include assessing patient care needs and eligibility, connecting them with community services, and collaborating with patients, caregivers, and fellow healthcare professionals.
Whether you're based in an office, out in the community, or in a healthcare facility, you'll be at the forefront of providing comprehensive and patient-centered care. Plus, you'll be part of a supportive team of over 8,000 healthcare professionals.

- Collaborate with patients and their families to assess care needs, determine service eligibility, and create personalized care plans
- Connect patients with relevant service providers
- Oversee and monitor the implementation of care plans
- Build supportive relationships with patients and families
- Ensure patient needs align with available resources, respecting their values and preferences
- Active membership with the appropriate regulatory body:
- College of Nurses of Ontario
- College of Physiotherapists of Ontario
- College of Occupational Therapists of Ontario
- Ontario College of Social Workers and Social Service Workers
- Recent experience in community health or a similar field
- Familiarity with healthcare systems and community resources
- Strong interpersonal, communication, assessment, problem-solving, and decision-making skills
- Efficient time management, prioritization, and organizational abilities, both independently and in a team setting
- Accurate completion of necessary documentation, reports, and forms
- Possession of a valid driver's license and reliable vehicle
- Proficiency in a Windows environment
- Advanced fluency in English and French, both written and spoken
- Compliance with our mandatory COVID-19 vaccination policy
- Experience with diverse patient groups such as multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, or pediatrics
- Background in case management or related community roles
- Proficiency in French or another secondary language
