    Description

    Our organization is seeking an experienced Care Coordinator to join our team.


    This role involves assessing and determining patient care needs, providing access to community services, and engaging with patients, caregivers and healthcare practitioners. As a key member of our Community Care Team, you will be responsible for managing a caseload of diverse patients who require connected, accessible, patient-centred care.



    Key Responsibilities:


    • Conduct comprehensive patient assessments using validated tools
    • Determine eligibility for health services and coordinate referrals
    • Collaborate with the multidisciplinary healthcare team to develop individualized care plans


    Requirements:


    • A registered nurse (BScN or diploma), physiotherapist (degree or diploma), occupational therapist (degree or diploma), social worker (degree in Social Work), dietitian (Degree in Foods and Nutrition)
    • Suitable professional experience preferably gained within two years
