We are excited to announce we currently have an upcomming opening for a professional, enthusiastic, compassionate *RAI MDS Coordinator *to join our team.

Responsible to the Director of Care, the RAI MDS Coordinator is responsible for the effective management of resources and staff in optimizing the RAI-MDS processes in the home.

*Duties include (but are not limited to): - Communicate regularly with RAI-MDS users and the Leadership team.

  • Ensures that proficiency testing is completed annually by all required individuals.
  • Orientate new employees in RAI MDS.
  • Attend all data submission and other relevant training provided by the Canadian Institute for Health Information (CIHI) of the Ministry of Health and Long Term Care (MOHLTC)
  • Review, prepare and submit RAI quarterly data to CIHI
  • Responsible to initiate, promote and maintain therapeutic and rehabilitative (where possible) programs for each Resident.
  • Generate reports and, as appropriate, review them with the leadership team, registered nurses and front line staff
  • Ensure the resident assessment schedule is maintained, completed, current and accessible to all RAI users
  • Assist RAI users to correctly code assessments and provide support for the validation of coding
  • Oversee the RAI process according to home policies, MOHLTC regulations, College of Nurses Standards of Practice and InterRAI requirements
  • Ensure MDS assessments, resident assessment protocols and care planning are completed according to established timelines
  • Oversee the development and coordination of the data submission process
  • Generate RAI MDS 2.0 reports and facilitate their use in clinical and operational decision making
  • Lead the interdisciplinary team in assisting residents to maximize their remaining strengths and abilities in order to attain or maintain the optimum level of function possible or desired by the resident
  • Evaluate RAI MDS data and set goals, develop plans and monitor the effectiveness of restorative interventions focused on individual residents
  • Ensure that all nursing rehab care documentation is complete and reflects current assessed needs of the residents
  • Participates in the Quality Improvement Program of the Home.
  • Completes any other duties as assigned by the DOC
*QUALIFICATIONS:

Formal:

  • Current registration with the College of Nurses in Ontario
  • CIHI Experience:
  • RAI-MDS credential education or equivalent
  • Proven experience and skill in planning, coordinating and supervising RAI-MDS and Nursing Rehab care.
  • 23 years experience in Long Term Care preferred

Personal:

  • Reliable and able to maintain confidentiality.
  • Proven written and oral communication skills.
  • Good organizational ability
  • Excellent computer skills
  • Strong analytical and assessment skills
  • Ability to lead training and education with Adult learners
  • Proficiency in use of Computer programs, Microsoft Word, Excel and Outlook.
  • Must be fully vaccinated with two doses for COVID1
  • Must have a clear valid vulnerable sector check.
  • 2Step TB test completed within one year of hire date.
