    Description
    The Primary Care Team is a regional resource to 28 First Nations Communities and the community of Sioux Lookout.

    The Primary Care Team brings together a diverse group of health care professionals to work together collaboratively to help meet the individual and complex needs of clients.

    They deliver primary care to help clients with the specific health care goals and needs, ranging from managing chronic disease, to addressing complex health needs, to helping a client deal with a life challenge that is negatively impacting their health.

    These teams also empower clients and their families to be active participants in working towards their health care goals.

    Under the Sioux Lookout Area Primary Care Team, the Registered Nurse (RN) will be responsible for supporting the provision of primary care nursing services, including assessments, education, treatment and follow-up.

    The RN will work alongside the Primary Care Team, caregivers and community resources to ensure high quality and timely health services/interventions using culturally appropriate practices and best practice standards of care.

    The RN is a key part of the client circle of care and will work to their full scope of practice.

    Responsibilities will include the provision of safe and competent care through the application of professional knowledge; technical aspects of professional practice and the demonstration of compassion, professionalism and respect.

    The RN will work in both the clinic environment and communities; Baccalaureate Degree in Nursing BScN from an accredited University with identifiable community health nursing content
    Minimum of two (2) years' work experience as a Registered Nurse
    knowledgeable of First Nations' people, history, culture, health priorities and social issues
    Knowledge and proficiency in current evidenced-based methods and practice of primary care delivery, with an emphasis on health promotion and risk reduction
    A strong understanding of the social determinants of health and the impact on First Nations is required
    Knowledge and sensitivity to First Nations culture as well as the government health policies and regulations as they apply to First Nations
    Experience and training in First Nations cultural competency
    Ability to work collaboratively within the Primary Care Team other health care professionals
    Strong computer skills and previous experience with Microsoft Office
    Ability to function effectively during change management; Must have a valid Driver's License
    Must be willing to relocate and live in Sioux Lookout (or within a daily commuting distance)
    Ability to communicate in one of the First Nations dialects of the Sioux Lookout region is an asset.
    Previous experience with an Electronic Medical Record would be an asset
    Previous experience with Diabetic Foot Care would be an asset
    Previous experience with Wound Care would be an asset
    Previous experience with Hepatitis C Care would be an asset

    Provision of patient assessments, utilising clinical reasoning skills and manual assessment techniques to provide a physiotherapy diagnosis of their condition.

    Provision of Treatment plans that will enable patients to reach their full functional and work integration potential, in collaboration with all stakeholders (Patient, Primary Care Team members, Mental Health workers, etc)
    Work with other health providers to formulate prognosis and recommend best course of intervention.
    Maintain documentation as per college guidelines and SLAPCT policies and procedures

    Provide information to help facilitate understanding and access to the assistive devices, program and other funding initiatives for special equipment.

    Work collaboratively with community based service providers and agencies to meet identified needs and enhance delivery of care.

    Educate and counsel family members and caregivers regarding the impact of disability, injury or disease on the individual and their role in the treatment and recovery process.

    Educate and counsel to promote function and independence including health promotion and injury prevention.
    Collaborates with other health care professionals and community resources to promote comprehensive and coordinated care.
    Recommends and arranges for specialist referrals and appointments in consultation with physicians and other health care providers.

    Advocate on behalf of clients for service, equipment, navigation and funding including provincial services, Non-Insured Health Benefits (NIHB) and/or Jordan's Principle processes.

    Exercises care and caution in protecting confidential and sensitive information related to clients and personnel.
    Maintain documentation as per college guidelines and SLAPCT policies and procedures.
    Experience with children and adults who have multiple needs is an asset

    Assistive Devices Program (ADP) Authorizer status for mobility devices or experience with seating and mobility devices is an asset.

    Continuous quality improvement activities.
    Completes reports after each community visit (i.e.# of community members seen, planning/clinical meetings, meetings with leadership).
