The Community Transition Services Manager is responsible for Transition Services in acute, community and post-acute settings in the Edmonton Zone.

Transition Coordinators are considered complex case managers and assess patients' needs in the context of the whole person including their life situation and determinants of health.

Critical thinking and accurate interpretation of complex information from a variety of sources are essential to support professionals in the clinical decision-making required for the determination of appropriate continuing care living options for individuals in the Edmonton Zone.

The role of the Transition Coordinator is highly complex and includes comprehensive assessment, planning and coordination of care, as well as ongoing communication with other health-care providers across the continuum of care.

Managing this team includes removing barriers and creating partnerships to facilitate efficient and accurate assessment of individual's needs.

This position liaises daily with Acute and Continuing Care Managers and other community resources, providing information and support to Transition Coordinators clients, families, and caregivers.

The Transition Coordinator works within a multi-disciplinary team of health professionals and is responsible for appropriate referral to other programs/services, appropriate documentation, participation in conferences and team meetings, process, and outcome evaluation activities which the Manager is accountable.

There is a strong focus on patient and family centered care, the development and monitoring of staff and material resources.

Supporting staff individually and within teams is fundamental to this position. Strong communication skills facilitate the internal function of this role and promote and maintain essential external partnerships.

The incumbent will provide strong leadership and mentoring to front-line staff, manage resource allocation within Transition Services, and participate in network/program area management processes and meetings.

Required Qualifications:

Experience managing day-to-day operations of staff at the operations level monitoring the dynamic workload leveling and challenges.

Experience fostering client centered care, recognizing unique client situations and complexity of transition services in community, acute and post-acute settings.

Experience with issues management and following up on patient and family concerns. Ability to audit that services are delivered in accordance with team targets, legislation and AHS provincial policies and protocols. Collaborates with staff to resolve complex client/family issues/risk. Reports issues to the Director which impact the network/program area or the zone. Makes operational decisions within the area of responsibility based on clinical information, workload data and fiscal resources. Promotes internal and external partnerships enhancing effective communication related to areas of responsibility. Fosters an atmosphere conducive to encouraging and meeting staff development needs and student learning experiences. Monitors and documents staff attendance and performance. Provides constructive feedback while working with the staff to facilitate ongoing quality performance. Participates in the recruitment process, identifying needs to facilitating/providing orientation for new hires.


Additional Required Qualifications:

Bachelor's degree in health related field required (Masters preferred). An equivalent combination of experience and education will be considered. A minimum of 5 years of clinical practice or health related experience required (current community or continuing care preferred). Previous management experience in a health-related field is required. A valid driver's license and a vehicle are required.

Computer literacy in Microsoft Office (Word, Outlook), ePeople/Recruitment Management System, and Environment for Scheduling Personnel (ESP) is required, Meditech and Connect Care are preferred.

Annual membership in good standing with an accredited professional association is required.


Preferred Qualifications:

Knowledge of community resources is an asset.
