Job Description
In this community nursing role, you as a Licensed Practical Nurse (LPN) Team Lead will have scheduled visits within the community to care for patients in various settings with the support of RNs for more complex patients. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.   • Registration with the CLPNA as a Licensed Practical Nurse in good standing
  • Have current or previous team lead or management experience
  • Experience in delivering community care is an asset
Reliability and flexibility Willingness to travel to and from patient residences within a designated area Experience working with the vulnerable population Passion for making a difference in your community Required oncall coverage, including after hours to ensure patient safety and to meet patient care needs Can work 3PM 11PM Thursday Monday with Tuesday, Wednesday Off (FLEXIBLE)
