John Howard Society of York Region (JHSYR) is a non-profit, charitable organization serving residents of York Region. The organization works with individuals involved in any stage of criminal justice involvement and their families.


Home Again provides wrap-around case management supports to youth ages 15-29 who are homeless or precariously housed with an enhanced focus on youth with justice and welfare involvement.

The program addresses the need for acquiring stable housing, employment support, establishing support systems, social and emotional learning, family reunification, financial literacy, access to basic needs (i.e. food and rental assistance), and life skills. Youth are supported with self-directed goal planning, housing supports, access to basic needs, and referrals to community supports.


The function of this position is to provide case management support to youth who are homeless and precariously housed in York Region.

The caseworker will conduct intake assessments; needs assessments; goal planning and learning activities; as well as provide housing support and referrals to community resources.


JOB DESCRIPTION:

  • Conduct intake assessments and needs/risk assessments with youth
  • Conduct 1:1 sessions with youth to support with social and emotional learning, life skills, financial literacy, and coping skills
  • Maintain ongoing case management appointments and followup calls with youth
  • Support individuals with immediate temporary housing and help them secure safe, stable, permanent, and appropriate housing
  • Complete goal planning exercises and various learning activities with youth based upon their individual needs
  • Work with family members requiring support for their loved one's in order to focus on the wellbeing of the entire family unit
  • Maintain partnerships with community agencies and engage in collaboration and partnership development, as directed by the Program Coordinator
  • Complete statistical data collection, client intake statistics, and financial ledgers
  • Administer program evaluation surveys
  • Maintain membership on committees and working groups, as directed by the Program Coordinator
  • Adhere to directives and program standards of John Howard Society of York Region
  • Engage in continuous learning as directed by the Program Coordinator
  • Attend staff and team meetings
  • Accommodate day and evening hours

QUALIFICATIONS:

  • Post-Secondary education in related field (i.e. Social Work, Criminology, Addictions and Mental Health, Child and Youth Care, etc.), or an equivalent combination of education and practical experience
  • Experience working with criminal justice and child welfare involved youth and families, with an understanding of the barriers and precipitating factors leading to criminal justice and child welfare involvement
  • Experience in prevention and diversion initiatives that reduce and/or stop the inflow of persons becoming homeless
  • Lived experience of involvement with the criminal justice involvement, welfare involvement, substance use, the mental health system and/or homelessness is an asset
  • Professional demeanour with strong rapport and relationship building skills
  • Ability to actively listen and provide an empathetic approach to client interaction
  • Knowledge of goal planning and mental health and wellness activities with the ability to seek out new learning activities when needed
  • Ability to maintain relationships with community agencies and stakeholders
  • Strong administrative and organizational skills and abilities
  • Knowledge of community resources throughout York Region with the ability to seek out new community resources as needed
  • Knowledge of crisis intervention techniques
  • Approach work through a traumainformed and clientcentered lens
  • Excellent interpersonal and communication skills
  • Proficiency in Microsoft Office programs and online meeting platforms
  • Ability to work with diverse populations and communities with an awareness of socioeconomic and language barriers
  • Ability to speak a second language is an asset
