Job Description
The (SHA) - The Bridge Clinic is a temporary medical home designed to be the connector of care until care needs stabilize or a permanent practitioner can be found. Referrals are accepted from acute care in phase 1 with predicted expansion to walk-in clinics and 811. We don't have the resources to provide full service primary care to the entire population. We decided to "cherry pick" the people most in need of care optimization. We will start by accepting referrals from acute care. Our hope is that the assurance of robust follow up care will help acute care practitioner's transition patients to community based care in a timely manner.
Our plan is to optimize care so well that we can transition Bridge Clinic patients to new family physicians setting up practice. The Bridge Clinic will be a test medical home for us, and an important part of network development. We see the importance of intentionally linking medical homes and networks to avoid creating siloes in primary care. We anticipate that the Bridge Clinic will give us valuable insights to help inform future development.
The team consists of a Family Physician, Nurse Practitioners, Primary Nurses and Assessor Coordinator. Our plan is to provide a coordinated initial appointment with the physician and PHC nurse with follow up provided by the Nurse Practitioner. Patients will experience a coordinated model of care in order to stabilize their health through with linkages to other health services. The Nurse Practitioner will take on the role of the MRP. Using a client centric approach, patients of the clinic will have access to PHC service such as CDM, therapies, pharmacy, MHAS, wound care to name a few. Assessor/coordinators & Nurses will provide system navigation to health resources in the community. Providers will support patients across 3 broad service pathways as highlighted above.

Saskatoon, Saskatchewan - The Bridge clinics will deliver services in leased spaces throughout the city, ideally with one Bridge clinic per network to support closer to home network based services. Service delivery will not disrupt existing family practice flow. Our goal is to enhance capacity and access to PHC for the patients who require follow-up. Bridge clinics will be offered in 4 hours blocks in leased clinic spaces across Saskatoon 7 days per week. Operational hours are to be determined once clinic space is affirmed.
Job Qualifications
Successful applicants will be eligible for a provisional or regular license with the Candidates must obtain privileges with the SHA to practice family medicine in Saskatoon.
