What is the Guelph CHC?


The Guelph CHC is located in the vibrant city of Guelph, Ontario, and provides primary care services and community health programs using an interdisciplinary, team-based approach.

We have innovative client-centered services delivered by interdisciplinary teams.

Our clinical team consists of family physicians, nurse practitioners, and registered practical nurses, who work collaboratively with allied health workers, including social workers, dietitians, outreach workers, and cultural interpreters.


The Guelph Community Health Centre works within a determinants of health framework which emphasizes health promotion and education, disease prevention, anti-oppression, and recognition of special risk factors (social, physical, psychological, etc.) which may affect the health of clients.


This position is located primarily at the downtown site of the CHC with the possibility of some work from home or offsite time.

Primary Responsibilities:


  • Provide comprehensive primary care services, including assessment and treatment, preventative care, supportive client counseling, prenatal and neonatal care, and clinical consultation with Nurse Practitioner colleagues
  • Work within a determinants of health and equity framework which emphasizes health promotion and education, disease prevention and recognizes the special risk factors (social, physical, psychological etc.) may affect the health of clients
  • Participate in the CHC's on call service as required (consisting of afterhours phone availability in rotation with other physician colleagues) and occasional home visits as necessary
  • Ensure high quality and safe care is delivered by maintaining complete and accurate medical records, participating in chart reviews and support the implementation of new guidelines and directives
  • Maintain professional development through continuing education, including CHCsupported Problem Based Learning opportunities
  • Collaborate with interdisciplinary teams on client care plans

Qualifications:


  • Licensed Family Physician, as per the requirements of the Ontario College of Physicians and Surgeons, with CCFP certification
  • Possess or is eligible to obtain required malpractice coverage with CMPA (this coverage is eligible for reimbursement)
  • Experience working with communities and individuals who face barriers accessing health care services (e.g. clients experiencing language barriers, mental health concerns, using street drugs etc.)
  • Previous experience in community health/social services and/or a valuesbased organization is an asset
  • A demonstrated commitment to health equity and alignment with the CHC model of care
  • Excellent communications skills, both verbal and written, along with the ability to prioritize and multitask
  • Ability to work interdependently and respectfully in an interdisciplinary team environment
  • Identifying as a member of an equity deserving group and/or proficiency in other languages is an asset
