Ambulatory Services' Adult Day Program (The Samuel Lunenfeld Moutainview Club)has an opportunity for aPROGRAM ASSISTANT, PERMANENT FULL TIME
SEIU Service Hourly Pay Rate:
$ $24.1549
The Adult day Program is focused on offering older adults a wide range of social, cognitive, physical and spiritual activities. This program is designed for community dwelling seniors with physical frailty as well as moderate to severe cognitive impairments.
Responsibilities include but are not limited to:
- Providing safe and effective direct care and support during programs in order to meet the needs of program members as identified in the interdisciplinary care plan
- Supporting the independence, dignity, social, emotional and cultural wellbeing of program members
- Promoting an environment that is conducive to dementia care, mental health care and physical frailty in a community setting
- Supporting the delivery of inperson and virtual programs related to health and wellness of Program Members
- Planning and conducting assigned programs as well as initiating new programs and planning of special events under the guidance of the Therapeutic Recreationist team
- Supporting members being transported to and from the program while on the Baycrest bus
- Initiating and maintaining relationships with the client, their family and other care providers
- Some evenings and weekends may be required.
Qualifications include but are not limited to:
- Current P.S.W. certificate from a recognized community college or educational institute or equivalent combination of education and experience
- Experience in program activation/ recreation preferred
- Activation Diploma preferred
- Current Basic Cardiac Life Support (BCLS)
- Certificate in Dementia Care
- Ability to work independently and as a part of the interdisciplinary team, and effectively managing challenging situations including the behavioural and psychological symptoms of dementia
- Excellent interpersonal, communication, organizational and problem solving skills
- Experience working with atrisk or frail seniors living in the community preferred
- Experience delivering services and care to seniors with cognitive impairment, mental health issues and complex medical issues
- Experience working in a Day Centre or community service setting preferred
- Knowledge of Yiddish, Hebrew, Russian languages an asset
