**Position Summary
The Patient Care Manager, Outpatient Mental Health, has operating responsibility for those services within the Mental Health & Addictions Program that see patients on an outpatient basis and have strong linkages in the community.
Our new Patient Care Manager should be someone who is passionate about mental health care and who embraces quality and process improvement, and believes in partnerships to advance and develop innovative care models and strategies to ensure exceptional care we provide to a vulnerable and growing patient population.
Working in partnership with the Medical Service Head of Outpatient Mental Health, the Patient Care Manager will oversee a dynamic team of nursing and allied health staff.
We are looking for a competent change leader, with a passionate curiosity about quality improvement and excellent skills in engaging staff, physicians and internal and external stakeholders around implementation of new programs and services.
This position also works closely with the Patient Care Managers and Medical Service Heads/Directors for the various service areas.Qualifications:
- Masters Degree in a Health related field required or willingness to complete within 2 years of hire.
- Current registration with a Regulated Health Professional College preferred
- Three to 5 years demonstrated leadership experience required
- Strong clinical background in Mental Health required
- Demonstrated strong financial and human resource management skills.
- Demonstrated ability to foster partnerships to advance patient and family centred care.
- Demonstrated evidence of innovative leadership and critical thinking, with proven ability to foster a collaborative team and patient/family approach to decision making.
- Demonstrated evidence of continuing professional development
- Demonstrated aptitude, familiarity and comfort working with a diverse community
- Demonstrated ability in communication, team building, problem solving and change management
- Proven success in managing change and fostering a positive workplace culture.
- Proven ability to develop and maintain high volume effective services for people with a serious mental illness
- Satisfactory attendance record.
