What does the Community Support Services team do?


Our Community Support Services team, which is primarily funded by governments, provides essential services that enable people to live as independently as possible.

Whether the disability is physical, cognitive or the result of brain injury or aging, our team of 1,600 employees assists people in their homes and communities, helping them gain a greater level of functioning and connection with others.

Over the past year, the number of people we serve rose to more than 6,100 people.


This Opportunity


We are currently looking to fill one Rehabilitation Services Coordinator - One Permanent Full Time Position 35 Hours per week on Winnipeg.

Starting wage at $28.43 per hour.


Duties include the following:


  • Plans and implements programs designed to enhance the efficiency and safe delivery of Community Support Services to clients with various disabilities.
  • Effectively manages and approves program resources, human and financial needs to meet program goals and objectives
  • Recruits, employs, trains, supervises and evaluates support service staff, within a predetermined budget in accordancewith the approved program plans.
  • To coordinate the assessment development, implementation or care plans.
  • Monitorsand evaluatesoperational effectiveness, and effects changes required for improvement.
  • Participates as a member of CSS administration team in planning, problem resolution, quality improvement and review of service delivery performance.
  • Act as a resource to staff and families in the provision of longterm support and service coordination for complex to enable adults with disabilities to remain in community.
  • Act as a staff resource to support activities of Community Support Supervisor/Program Manager
  • Valid drives license and access to vehicle
  • Ability to travel regularly between MODC programs and the community of Winnipeg, Manitoba
  • Compensated Oncall on a rotational basis

Background / Qualifications:


  • Bachelor's degree in Human Services program or equivalent preferred
years' experience in the Social Services/Health Sector, coordinating services for individuals with a disabilities.

  • Knowledge of cognitive impairment and mental health diagnosis
  • Community based experience working with individuals with a cognitive impairment, physical disabilities or special needs
  • Strong written and verbal communication skills.
  • Highly organized and excellent time management skills
  • Demonstrated ability to supervise and train employees to include organizing, prioritizing and scheduling workassignments
  • Problem solving and conflict resolution skills
  • Knowledge of human relations
  • Knowledge of behaviour management principles
  • Knowledge of Health & Safety legislation
  • General understanding of computer hardware and software
  • Strong ability to assess and determine effective compensatory strategies to support maximum community participation/inclusion.
