The Dorothy Ley Hospice (DLH) offers both community and residential care to people living with a life-limiting illness or loss.

Our goal is to enable individuals to live their life to the fullest in comfort, with dignity, meaning and hope.

Our services, offered at no cost to the individual or their family members, address their physical, emotional, spiritual, and practical needs.

Last year, Hospice staff and its more than 200 trained volunteers served over 2,500 individuals.


Position Mandate:


Registered Nurses in the specialist PPSMC role offer palliative care education, consultation, mentorship, and links to resources for front line health service providers across the continuum of care with a focus on partnering to build palliative and end-of-life care capacity in the community.

The position requires flexible hours, including evenings. This position reports to the Director, Palliative Care, Pain, and Symptom Management Consultation (PPSMC) - Toronto Service.


Key areas of responsibility:


  • Deliver casebased consultation, education, training and mentorship to front line service providers supporting a palliative care approach in a variety of care settings.
  • Facilitate palliative care courses, seminars and other education opportunities using principles of adult education
  • Leadership in the development and sustainability of palliative care teams; participation at rounds and case conferences as appropriate; links clinicians to palliative care resources and supports as needed
  • Liaise with all sectors of the health care and social services community.
  • Participates on regional advisory committees
  • Develop quality training initiatives, strategic plan implementation, attain yearly program objectives
  • Collect and report relevant statistics
  • Involvement in interdisciplinary policy and procedure development
  • Partner in project and research initiatives
  • Continuous professional development
  • Participate in accreditation processes as appropriate
  • Palliative Care Consultant Network (PCCN) membership
  • Perform other duties as assigned

Qualifications & Experience:

Registered Nurse (RN, APN, or RN(EC)) in good standing with College of Nurses of Ontario (CNO)

CNA Certification in hospice palliative care (CHPCN(C)) or equivalency in knowledge, skill and experience

CAPCE and LEAP certificate

RNAO Membership

Minimum five (5) years of nursing experience in palliative care or chronic progressive illness

Current clinical knowledge in pain and symptom management

Expertise in facilitating adult education for a variety of health disciplines

Demonstrated knowledge of health care system as it relates to palliative care delivery

Excellent interpersonal, presentation and communication skills

Demonstrated ability to work independently with proficient time management

Demonstrated commitment to effective teamwork

Demonstrated problem solving and critical thinking skills

Ability to successfully interface with all levels of healthcare professionals

Experience in program development and coordination considered an asset

Demonstrated commitment to professional development

Sensitivity and awareness of culturally and racially, economically and socially diverse communities

MS Office Suite skills (Outlook, Word, Excel, PowerPoint)


Significant Working Conditions:

Ability to work flexible hours including evenings and weekends to meet the needs of the program

Valid Ontario driver's license with own vehicle for transportation to a variety of care settings

Home Office capability
