Job Description:


As a member of a multi-disciplinary team in a Program for Women and Children, you will provide a range of social work services based on family-centred care.

This care includes perinatal loss; child protection concerns for the newborn, neonate and paediatric patient; crisis intervention counseling for parents and/or the obstetrical patient, and support to families facing birth anomalies, and acute and/or chronic illness.

You will advocate on behalf of the patient and liaise with the multi-disciplinary team and community agencies to access services and provide continued health care as identified.

As a member of the Women's and Children's team you will attend team meetings, consult with your professional peers and represent WOHS as required.

You will report to the Manager of Women's and Children's Program, with professional accountability to the Professional Leader for Social Work.

You will be working closely with the other program social worker to cover the needs of all clients in the Women's and Children's services.

The Social Worker will also be an integral part of the Corporate Social Work team, participating in yearly strategic planning initiative and corporate professional development.

This role is integrated into the Paediatric Lifestyle Team, supporting Ontario Bariatric Peadiatric Network (OBPN), Intake and Activity Nights.


Qualifications:


  • Masters Degree in Social Work required
  • Three to five years experience in a healthcare environment
  • Currently registered and in good standing with OCSWSSW
  • Knowledgeable in community resources
  • Knowledge and experience working with relevant legislation (ie: SDM Act, POA, Consent and Capacity, etc.)
  • Knowledge of the psychosocial impact of chronic illness on the person and the family
  • Experience in providing end of life care and bereavement counselling
  • Experience in women's and children's is preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently and evaluate patient and family needs
  • Ability to work with community partners such as Children's Aid Society
  • Excellent organization skills and ability to set priorities as required
  • Excellent critical thinking and problemsolving skills required
  • Ability to work as part of a multidisciplinary team
  • Must demonstrate Osler's Values of Respect, Excellence, Service, Compassion, Innovation and Collaboration
  • Excellent attendance, punctuality and work record
  • Computer proficiency required
  • May be required to travel between and work at all Osler sites
  • BCLS certification required
