    What is Pay Per Visit?

    Responsibilities:

    • Conduct client assessments, develop care plans, and provide care based on best practices
    • Act as a primary care lead for client caseload
    • Support clients through care transitions

    Requirements:

    • Registered with the College of Nurses of Ontario
    • CNA specialty certification an asset
    • Paediatric experience preferred
    • Passion for community care and evidence-informed practice
    • Valid CPR certificate, driver's license, and vehicle
    • Updated immunization records and clear background check
    • Availability for varied shifts and physical requirements
