Summary


In support of the mission and vision of Lambton Elderly Outreach, the incumbent will be responsible for the training and support of field staff across several community support services and programs.

The Manager will be responsible for the hiring and training of relevant staff, ensuring that all work is completed following safe work practices.

It is expected that the incumbent will work alongside the Program Coordinators to ensure that the needs of the clients and/or their caregivers are satisfied.

Core Competencies

  • Computer skills in programs such as Microsoft word, Excel, and Databases
  • Working knowledge of InterRAI assessment tools, AlayaCare and NOVUS databases is an asset
  • Ability to communicate effectively in English (written and verbal)
  • Ability and flexibility to work independently on a range of tasks
  • Demonstrated ability to work in a team environment
  • Experience in working with seniors and person with disability
Job Duties

  • Collect and maintain client and employee information based on program needs and employer requirements in accordance with Lambton Elderly Outreach's confidentiality/privacy policy, and PHIPA and PIPEDA legislation
  • Document accurately on each client according to the agency guidelines, and statistical requirements
  • Communicate with clients, family and caregivers as required
  • Reviewing of assessments entered by staff, ensuring they are complete and accurate
  • Inhome assessments for high need clients
  • Tracking of quality assurance measures
  • Training and supporting unregulated care providers in the field and office
  • Timesheet reconciliation
  • Incident reporting and follow up
  • Investigating client concerns and complaints
  • Attending meetings with community partners as required
  • Completion of staff performance reviews
  • Participation in after hours oncall rotation

Requirements:


  • Current RPN certification in good standing with the Ontario College of Nurses
  • Postsecondary education in social services, gerontology, or equivalent
  • Minimum 3 years' experience. Preference will be given to those who have delivered community support services or those having experience in the health care field.
  • Completion of a satisfactory Criminal Reference Check and Vulnerable Sector Screen
  • Strong organizational and administrative skills
  • Ability to quickly assess and respond appropriately to situations
  • Tact, discretion, honesty, and rapport with seniors and people with disabilities
  • Comfort providing care to clients in their own homes
  • Ability to safeguard and maintain a high level of confidentiality of client information
  • Valid driver's license, safe driving record, and access to a vehicle
  • Current CPR and First Aid
  • Experience working effectively with culturally and economically diverse populations
Working Conditions

  • Working in an office environment and client homes
  • Involves regular use of standard office environment with exposure to irritants such as background noise, telephone sounds, and equipment noise
  • The position may involve sitting over moderate time periods
  • Occasional travel is also required to attend training and/or meetings outside of Lambton County
  • Dealing with difficult or demanding clients or situations may be part of a working day
  • Accountability
  • Good judgment is required to determine the most appropriate methodology to complete assigned tasks and to ensure appropriate handling of sensitive and confidential information
  • Information technology work is of a highly technical nature performed independently under general supervision
