The Edmonton Southside Primary Care Network was the first PCN in Alberta.

We are currently seeking an experienced full-time,
temporary,
Central Social Worker to join our team This will be a 12-month contract with the possibility of extension.


Position Overview:


The Central Social Worker (CSW) will provide comprehensive social work services and care to patients within the Primary Care Network.

The CSW will assess, plan, implement and evaluate social work care plans for patients who require direct practical support.

This position will support referred patients to identify and access appropriate services, through collaboration with other professionals within a multi-disciplinary team.

The CSW will be an integrated team member working to their full scope of practice within the primary care setting.


The Central Social Worker will work with family physicians in the community, other health professionals and local and regional programming in a multidisciplinary environment to ensure a high quality of evidence based health care to the practice population.

The Central Social Worker will report to the Central Office Primary Care Manager.


Key Roles and Responsibilities:


  • Completing a thorough (bio)psychosocial assessment of referred patients/families and work with the patient/family to prioritize needs
  • Assist patients/families to access financial support, housing, employment opportunities and other resources that would improve the patient's quality of life
  • Support patients and families to navigate health and social systems, and advocate for them within those systems when required
  • To work collaboratively with physicians in a multidisciplinary team across primary care practice settings
  • To participate in clientcentered and culturally appropriate care of individual patients, practice populations and associated communities
  • To provide social work services and care to patients in the family practice setting
  • The Central Social Worker will assess, plan, implement and evaluate care and work with patients with complex health care needs
  • To make professional, competent and autonomous decisions with patients as a member of the multidisciplinary team
  • To act as a social work resource to other multidisciplinary team members, healthcare institutions and community agencies or programs
  • To ensure accurate, objective and timely clinical documentation in the appropriate medical record.

Experience, Skills and Qualifications:


  • Minimum Bachelor's Degree in Social Work
  • Registration with Alberta College of Social Workers
  • Current Basic Life Support (BLS) for Healthcare provider
  • Vehicle/license required
  • Previous experience:
  • Primary care setting
  • Electronic medical records
  • Group facilitation/instruction
  • Health behavior change methods (i.e., Motivational Interviewing)
  • Excellent written and oral communication skills
  • Understand the Alberta privacy laws and act according to those laws
  • Should behave responsibly, be accountable and have excellent problem solving/critical thinking skills
