Position Summary

  • The Nephrology Social Worker is responsible for providing comprehensive, coordinated social work support to all kidney care patients in the RKCP SM and their families. The Nephrology Social Worker is an active member of the multidisciplinary care team and is responsible for providing consultation to the team.
  • PRIMARY RESPONSIBILITIES:
  • Providing comprehensive biopsychosocial assessments for patients in the RKCP SM.
  • Providing information and education to patient and their families on treatment options for chronic kidney disease, including comprehensive conservative renal care.
  • Providing counseling services to patients and families coping with the effects and complications of chronic illness and dialysis, including adjusting to treatment and changes to lifestyle and quality of life.
  • Providing ongoing emotional support and counseling to patients and families for depression, anxiety, stress management, caregiver concerns and burnout.
  • Facilitating referrals to community resources such as transportation to medical appointments, housing, home supports, medical equipment, respite care and bereavement support.
  • Assistance with information, documentation and advocacy as needed for treatment related costs, income support, medication coverage and tax credits.
  • Providing education, information, support, and counselling to patients, families and staff about Advance Care Planning, Palliative Care and endoflife care.
  • Providing consultation to the multidisciplinary care team regarding biopsychosocial aspects of patient and family care, and orientation to new staff about the social work role and Advance Care Planning.
  • Maintaining complete and ongoing clinical records to be compiled on the medical chart.
  • Maintaining accurate statistical data.
  • Participating in Social Work professional development opportunities (workshops, seminars, conferences) and consultation with nephrology peers through the Canadian Association of Nephrology Social Workers (CANSW) listserve, Regional meetings and annual conference, and sharing learning with social work colleague.
  • Participating in hospital and community development and advocacy for the profession.
  • Participating in various capacities with the Kidney Foundation of Canada including financial assessments for last resort and comfort needs funding, promoting educational materials, and peer support to patients and families.
  • Participating in weekly Multidisciplinary Patient Care Rounds.
Performing other duties as may be assigned by the Manager of the RKC Program


Qualifications:


  • QUALIFICATIONS:

Education:


  • M.S.W. or B.S.W. required.

Experience:


  • Minimum three (3) years hospital experience required.
  • Registered member of the Ontario College of Social Workers required.
  • Registration with the Canadian Association of Nephrology Social Workers (CANSW) strongly recommended.

Competencies:

  • Demonstrated strong communication skills in English, both written and verbal.
  • Expert knowledge of hospital social work, evidencebased social work theory and practice, advance care planning, palliative care and community resources.
  • Good understanding of the Health Care Consent Act and Substitute Decision Act.
  • Demonstrated computer literacy skills.
  • Demonstrated ability to organize, prioritize and multi task with efficiency.
  • Demonstrated ability to work both independently and in a team environment.
  • Demonstrated ability to focus on detail.
  • Demonstrated strong presentation and facilitation skills.
  • Demonstrated ability to maintain and champion confidentiality.

Other:

  • Must have a valid driver's license and a reliable vehicle. Must be willing to travel to all sites within the RKCP.
