SERVICE COORDINATOR


Brampton Caledon Community Living, a recognized leader in developmental services supporting over 2000 individuals and their families, is seeking a full-time Service Coordinator to join our team.


The Service Coordinator will assist individuals with an intellectual disability and their families to access resources and services, promote family capacity, and liaise and coordinate support with other service providers, schools, and community agencies to achieve identified goals.


_ Qualifications/Skills Required:
_


  • A Bachelor's degree from an accredited university in a related discipline is strongly preferred or a DSW, SSW, from an accredited community college.
  • 23 years experience with Service Coordination
  • Experience supporting families with a family member with an intellectual disability, and knowledge of the resources and services relevant to families caring for a person with an intellectual disability.
  • Knowledge about relevant resources and services in the community, the developmental service system, developmental service legislation, government funding and resources, and capacity to consent to and substitute decisionmaking rules and regulations.
  • Knowledge of Special Education legislations, policies, and procedures
  • A proven track record in planning and working with families.
  • Strong interpersonal skills: the ability to express and present oneself in a professional manner; strong written and verbal communication skills; sound decisionmaking skills.
  • Demonstrated understanding of Social Role Valorization and Personal Outcomes Measures
  • Demonstrated ability to work effectively as a team member.
  • A flexible schedule involves occasional evenings and weekends.

Must Have:
_**- Valid driver's license Class G and a reliable car

  • Acceptable Driver's Abstract and personal liability car insurance
  • Current, Clear, Police Record Check & Vulnerable Sector Check
  • Current Standard First Aid/CPR & CPI
  • Proficiency in Computer skills
  • Fluency in French is an asset
