Collaborating as an interdisciplinary team, we provide services and supports that address the social determinants of health including factors and barriers such as poverty, inadequate housing, food insecurity, substance use, language and culture, as well as stigma and social exclusion.

The Health Centre works with the most complex client population served by primary health care professionals within Waterloo Region.

Our highly skilled and diverse team works from a strength-based approach with individual clients as well as the broader community.

We collaborate with many partner agencies and organizations within Waterloo Region.

Our team consists of Family Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians, Chiropodists, Pharmacists, Psychiatrists, Health Promoters, Community Health Workers, Administrative Staff and Volunteers.

The Health Centre is currently recruiting for the following position:

FOOD PROGRAM WORKER

Part-time Fixed-Term Contract 0.6 FTE - August 23, 2023 to June 28, 2024

Position Summary


The Food Program Worker will serve as the primary point of contact for the Food Market, oversee the daily operations of the program, and welcome food recipients and community partners to Community Healthcaring KW.


We value personal lived experiences with varying degrees of food insecurity, including experiences such as meal skipping, consuming lower quality or variety of foods due to financial constraints, or enduring extended periods without food.

By understanding these struggles firsthand, you bring a unique ability to empathize with and build meaningful relationships with those we serve.


Specific Responsibilities

  • Promote Social Prescribing of the food program within Community Healthcaring KW's Primary Care team and CHC programs.
  • Grow connections and opportunities for food justice collaboration between the Food Market Program and community partner agencies.
  • Participate in team/staff meetings and committee work as assigned to support both service delivery and organizational goals.
  • Support Food Program volunteers and help create meaningful volunteer opportunities.
  • Facilitate food-related groups that focus on life-skill development, capacity building, and social networking.
  • Establish and maintain community food system resources for staff and program participants.
  • Facilitate the gathering of food market demographic information and provide quarterly reports that will inform programs and services.
  • Collaborate with the Health and Wellness team to develop, implement and evaluate CHC food programs and services.
  • Develop and monitor operating budgets for assigned projects/programs and ensure expenditures remain within budget.
  • Create and post social media content to promote the Food Market Program
  • Work during both regular and extended hours (evenings/weekends) and in locations within Kitchener to be determined by Community Healthcaring K-W.
  • Work in a manner that ensures client safety and minimizes risk to clients, volunteers, and the Centre.
  • Work in a manner consistent with the Community Healthcaring K-W Vision, Mission, and Guiding Principles Work in a manner that demonstrates self-reflection and personal accountability for work performance.
  • Adhere to all Community Healthcaring K-W policies.
  • Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
  • Work in a manner that preserves privacy and confidentiality.
  • Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the Health Centre.
  • Work in a manner consistent with the Health Centre's policy and commitment to Diversity, Equity and Inclusion in all processes and practices.
  • Work in a manner that demonstrates self-reflection and personal accountability for work performance.
  • Contribute to the Health Centre's practices of hiring, orienting and training of staff.
  • Collect, analyze and report on data and relevant information as required.
  • Participate in research as determined by the Health Centre.
  • Adhere to appropriate infection control practices including those related to Covid -19.
  • Maintain status of fully vaccinated for Covid-19, including boosters as they become available.
  • Perform any other duties as
